七味三芎汤对髋部骨折术后DVT治疗效果及凝血功能的影响

Effect of Qiwei Sanxiong Decoction on DVT Treatment and Coagulation Function after Hip Fracture Surgery

目的:探讨髋部骨折术后DVT治疗中七味三芎汤的效果及对凝血功能的影响。方法:回顾性选取2017年5月~2020年5月中医骨伤科髋部骨折患者100例,依据治疗方法分为低分子肝素钙注射液结合七味三芎汤治疗组(结合治疗组)和低分子肝素钙注射液单独治疗组(单独治疗组)各50例,统计分析两组患者的中医症候积分、大腿周径、凝血功能、临床疗效、深静脉血栓、不良反应发生情况。结果:结合治疗组患者治疗后较治疗前的中医症候积分降低幅度、大腿周径缩小幅度均显著高于单独治疗组(P<0.05);结合治疗组患者治疗后较治疗前的血浆FIB水平降低幅度显著高于单独治疗组(P<0.05),血浆D-D水平升高幅度、APTT、TT、PT延长幅度均显著低于单独治疗组(P<0.05);结合治疗组患者治疗的总有效率94.0%(47/50)显著高于单独治疗组76.0%(38/50)(P<0.05);结合治疗组患者的深静脉血栓发生率、不良反应发生率8.0%(4/50)、12.0%(6/50)均显著低于单独治疗组26.0%(13/50)、30.0%(15/50)(P<0.05)。结论:髋部骨折术后DVT治疗中七味三芎汤的效果好,能够有效改善患者凝血功能。

Objective:To investigate the effect of Qiwei Sanxiong decoction in the treatment of postoperative DVT of hip fracture and its influence on blood coagulation function.Methods:100 patients with hip fractures in a hospital from May 2017 to May 2020 were retrospectively selected and divided into combined treatment group(low molecular heparin calcium injection combined with Qiwei Sanxiong decoction)and single treatment group(low molecular heparin calcium injection alone)on the basis of treatment methods,with 50 cases in each group.The integration of TCM syndrome,thigh circumference diameter,coagulation function,clinical efficacy,deep vein thrombosis and adverse reactions of the two groups were statistically analyzed.Results:After treatment,the reduction of TCM syndrome score and thigh circumference in combined treatment group were significantly higher than those in single treatment group(P<0.05).The decrease of plasma FIB level in combined treatment group after treatment was significantly higher than that in single treatment group(P<0.05),the increase of plasma D-D level,the extension of APTT,TT and PT were significantly lower than those of single treatment group(P<0.05).The total treatment efficiency of 94.0%(47/50)in the combined treatment group was significantly higher than that of 76.0%(38/50)in the single treatment group(P<0.05).The incidence of deep vein thrombosis and adverse reactions[8.0%(4/50),12.0%(6/50)]in the combined treatment group were significantly lower than those in the single treatment group[26.0%(13/50),30.0%(15/50)](P<0.05).Conclusion:Qiwei Sanxiong decoction has good effect in the treatment of postoperative DVT of hip fracture,and can effectively improve the coagulation function of patients.
