免疫球蛋白分别联合阿奇霉素与红霉素治疗儿童支原体肺炎的疗效对比 被引量:2

Efficacy Comparison of Immunoglobulin Combined with Azithromycin and Erythromycin in the Treatment of Mycoplasma Pneumonia in Children

摘要 目的:对比免疫球蛋白分别联合阿奇霉素与红霉素治疗儿童支原体肺炎(MPP)的疗效。方法:采用前瞻性随机试验方法,选择2019年1月~2020年5月某院收治的138例MPP患儿作为研究对象,按交替分组法分为A组和B组各69例。两组患儿均静脉注射免疫球蛋白,A组另接受红霉素治疗,B组另接受阿奇霉素治疗,比较两组患儿治疗前及治疗3周结束时血气分析指标[二氧化碳分压(PaCO_(2))、氧分压(PaO_(2))、血氧饱和度(SaO_(2))]、炎性因子水平[白介素-4(IL-4)、肿瘤坏死因子-α(TNF-α)],并统计比较两组治疗期间不良反应情况。结果:治疗3周结束时,两组患儿PaCO_(2)低于治疗前,PaO_(2)、SaO_(2)高于治疗前,且B组PaCO_(2)低于A组,PaO_(2)、SaO_(2)高于A组,差异有统计学意义(P<0.05);治疗3周结束时,两组患儿血清IL-4、TNF-α较治疗前低,且B组低于A组,差异有统计学意义(P<0.05);治疗期间,B组不良反应发生率(4.35%)低于A组(15.94%),差异有统计学意义(P<0.05)。结论:与免疫球蛋白联合红霉素相比,免疫球蛋白联合阿奇霉素治疗可改善MPP患儿血气分析水平,减轻机体炎性反应,且不良反应少。 Objective:To compare the efficacy of immunoglobulin combined with azithromycin and erythromycin in the treatment of mycoplasma pneumonia(MPP)in children.Methods:A prospective randomized trial was conducted in 138 children with MPP admitted a hospital from January 2019 to may 2020,and all patients were divided into two groups according to the alternate grouping method,with 69 cases in each group.Two groups of children were given intravenous immunoglobulin,then group A received erythromycin treatment,and group B received azithromycin treatment.Blood gas analysis indexes[PaCO_(2),PaO_(2),SaO_(2)],inflammatory factor levels[IL-4,TNF-α]were compared between the two groups before treatment and at the end of 3 weeks of treatment,and the adverse reactions during treatment were statistically compared between the two groups.Results:At the end of 3 weeks of treatment,PaCO_(2) of the two groups was lower than before treatment,PaO_(2) and SaO_(2) were higher than before treatment,and PaCO_(2) of group B was lower than that of group A,PaO_(2) and SaO_(2) were higher than that of group A,the difference was statistically significant(P<0.05).At the end of 3 weeks of treatment,serum IL-4 and TNF-αof the two groups were lower than before treatment,and group B was lower than that of group A,the difference was statistically significant(P<0.05).During the treatment,the incidence of adverse reactions of group B(4.35%)was lower than that of group A(15.94%),the difference was statistically significant(P<0.05).Conclusion:Compared with immunoglobulin combined with erythromycin,the treatment of immunoglobulin combined with azithromycin can improve the blood gas analysis level of MPP children,and reduce the inflammatory reaction,with less adverse reactions.
