地榆的临床应用及其用量探究

通过搜集古医籍及现代医家临证经验及临床应用,总结出地榆具有以下临床应用特点:汤剂剂量范围为:2.9~74 g,临床常用剂量9~30 g;丸、散剂常用剂量范围为:3~30 g;外用常用剂量范围为:3~60 g。结合疾病、证型及症状选择最佳剂量,如凉血止血、化瘀解毒治疗崩漏、甲状腺结节、乳腺增生、子宫肌瘤等妇科疾病时,常用10~30 g;凉血止血、解毒敛疮治疗胃溃疡、溃疡性结肠炎、便秘等消化系统疾病时,常用3~30 g,丸、散剂常用3~20 g;清热解毒、敛疮治疗银屑病、带状疱疹、烧烫伤等皮肤病时,常用10~30 g,外用常用10~60 g;凉血止血治疗肾盂肾炎、慢性肾功能衰竭、尿血等泌尿系统疾病时,可用12~60 g;凉血止血、清热解毒治疗肺结核、哮喘咳血、咯血等呼吸系统疾病时,常用9~30 g。根据所治疗疾病的不同,地榆有不同的配伍药物,如清热凉血止血时常配伍槐角、槐花、黄芩、白茅根、白及等;凉血解毒、利尿通淋时常配伍生槐角、白花蛇舌草等;化瘀止血时常配伍三七、蒲黄等;收敛止血时常配伍黄芪、党参、升麻等。

Based on the collection of the clinical experience and application in the ancient medical books and of modern doctors,it is concluded that garden burnet root has the following clinical application characteristics:its dosage range in decoction is 2.9-74 g,while the common clinical dosage range is 9-30 g,in pills or powders 3-30 g,and for external use 3-60 g.Its optimal dosage is determined according to the different diseases,syndrome types and symptoms.For example,when used for cooling blood and hemostasis,removing blood stasis and detoxifying to treat metrorrhagia,thyroid nodules,breast hyperplasia,uterine fibroids and other gynecological diseases,its common dosage range is 10-30 g.For cooling blood and hemostasis,detoxifying and astringing sores to treat gastric ulcer,ulcerative colitis,constipation and other digestive diseases,its common dosage range is 3-30 g,in pills or powders usually 3-20 g.For clearing heat,detoxifying and astringing sores to treat psoriasis,herpes zoster,burns and scalds and other skin diseases,10-30 g is commonly used,and 10-60 g commonly for external use.For cooling blood and hemostasis in the treatment of pyelonephritis,chronic renal failure,hematuria and other urinary system diseases,12-60 g can be used.For cooling blood and hemostasis,clearing heat and detoxifying to treat tuberculosis,asthma,hemoptysis and other respiratory diseases,9-30 g is commonly used.According to the different diseases to be treated,garden burnet root can be combined with corresponding Chinese medicines.For instance,for clearing heat,cooling blood and hemostasis,it is often compatible with Japanese pagodatree pod,pagodatree flower,baical skullcap root,cogon grass rhizome,common bletilla rubber,etc.For cooling blood and detoxifying,diuresis and freeing strangury,it is often compatible with unprepared Japanese pagodatree pod,hedyotis,etc.For removing blood stasis and hemostasis,it is often compatible with sanqi,cattail pollen,etc.For astringing sores and hemostasis,it is often compatible with milkvetch root,tangshen,largetrifoliolious bugbane rhizome,etc.
