摘要
目的:对基于社会建构理论的LEARNS模式进行本土化修订,构建基于指南的糖尿病健康教育路径,促进临床糖尿病患者健康教育路径的科学、规范与统一。方法:以《最佳实践指南实施的工具书》为指导,通过成立循证实践小组、完成基线审查、差距分析等步骤,最终制定本土化健康教育路径。结果:本研究选定1部指南构建的健康教育路径内容涉及患者从入院到出院以后、患者学习需求评估、效果评价、健康教育流程、工具等。结论:方案基于循证指南,结合医院具体临床情境、患者意愿和偏好以及医护人员专业判断形成,可为糖尿病患者健康教育提供参考和指导。
Objective:To establish diabetic health education path based on guideline,and promote the scientific,standardized and unified path of health education for clinical diabetic patients by making a localization revision of LEARNS model based on social construction theory.Methods:Guided by the reference book for best practice guide implementation,localized health education path was finally established through establishing evidence-based practice group,completing baseline review,gap analysis and other steps.Results:The study selected health education path built in the guideline,which involves patients from admission to discharge,learning needs assessment,effect evaluation,health education process,tools and others.Conclusion:The scheme forms based on evidence-based guide by combining with specific clinical situation of the hospital,the patients’willing and preferences and professional judgement of medical staff,and it can provide reference and guidance for health education of diabetic patients.
