超声及新技术应用在取环术前风险评估中的价值

Value of Ultrasound and New Technology Application in Risk Assessment before Ring Removal Operation

目的研究常规经腹法或联合法经阴道二维、三维超声检查在计划生育取环术前评估规避取环术风险的应用价值及不同操作者、方法学差异性比较。方法选取2018年4月至2018年6月我院取环术前超声受检者151例为研究对象,由不同操作者随机进行常规法(C组)或联合法(A组、B组)行术前超声评估分类节育环(IUD)与术后对照研究分析。结果 151例研究对象术前超声评估、超声判断分类及术前术后对照分类结果配对McNemar检验,P=1.000,说明术前超声技术IUD分类和术后对照差异没有统计学意义,超声术前判断准确度高;不同操作者、不同超声方法对比分析,即对联合法和经腹法分类结果进行卡方检验,P=0.044,说明两种方法差异具有统计学意义,联合法超声技术正确率更高。结论联合法特别是新技术应用经阴道三维显像在查环取环术前进行超声评估可凸显其优越性,有潜在的临床应用价值,提倡超人多使用该法及加强超声质控。

Objective To study the application value of conventional transabdominal or combined transvaginal two-dimensional and three-dimensional ultrasonography in pre-family planning to assess the risk of circumcision avoidance and compare the differences between different operators and methodologies.Methods A total of 151 patients undergoing preoperative ultrasound examination in our hospital from April 2018 to June 2018 were selected as the research objects,and different operators were randomly selected to perform conventional methods(group C)or combined methods(group A,group B).Analysis of preoperative ultrasound assessment and classification of birth control ring(IUD)and postoperative control studies.Results The results of preoperative ultrasound evaluation,ultrasound judgment classification,and preoperative and postoperative control classification of 151 subjects were matched by McNemar test,P=1.000,indicating that there was no statistically significant difference between preoperative ultrasound technology IUD classification and postoperative control,ultrasound preoperative judgment high accuracy;comparative analysis of different operators and different ultrasound methods,that is,the chi-square test of the combined method and the transabdominal method classification results,P=0.044,indicating that the difference between the two methods is statistically significant,and the combined method ultrasound technique has a higher accuracy rate.Conclusion The combined method,especially the new technology of transvaginal three-dimensional imaging,can highlight its superiority and have potential clinical application value by using transvaginal three-dimensional imaging before ring-checking and removing the ring.
