基于4R模式的延续性护理在中晚期原发性肝癌化疗患者护理中的应用效果

Application Effect of Continuous Nursing Based on 4R Model in the Nursing of Patients with Advanced Liver Cancer undergoing Chemotherapy

目的探讨中晚期原发性肝癌化疗患者应用基于4R模式的延续性护理的干预效果。方法按照随机抽样法,将2018年11月至2020年11月我院收治的中晚期原发性肝癌化疗患者500例分为一般护理组(250例,给予常规护理)与4R模式组(250例,给予基于4R模式的延续性护理)。比较两组患者的护理疗效、自我效能、生活质量。结果干预后,4R模式组护理总满意度明显高于一般护理组(P <0.05)。两组ESCA量表自我概念、自我责任感、自我护理技能、健康知识水平及总分均较干预前升高,且4R模式组ESCA量表各维度得分及总分均高于干预前及一般护理组,差异显著(P <0.05)。两组患者的QOL-LC量表心理功能、躯干功能、社会功能、症状/不良反应及总分均升高,且4R模式组QOL-LC量表各维度及总分均高于干预前及一般护理组(P <0.05)。结论中晚期原发性肝癌化疗患者应用基于4R模式的延续性护理的效果显著,可以提高护理满意度、自我护理能力及生活质量。

Objective To investigate the intervention effect of continuous nursing care based on 4 R mode in patients with advanced primary liver cancer chemotherapy.Methods According to the random sampling method,500 patients with advanced primary liver cancer undergoing chemotherapy in our hospital from November 2018 to November 2020 were divided into general care group(250 cases,given routine care)and 4 R model group(250 cases),give continuous care based on the 4 R model.The nursing efficacy,self-efficacy and quality of life of the two groups of patients were compared.Results After the intervention,the total satisfaction of nursing in the 4 R model group was significantly higher than that in the general nursing group(P<0.05).The self-concept,self-responsibility,self-care skills,health knowledge and total scores of the two ESCA scales were higher than those before the intervention,and the ESCA scale scores and total scores of the 4 R model group were higher than those of the pre-intervention and general care groups,the difference was significant(P<0.05).The psychological function,trunk function,social function,symptoms/side effects and total scores of the two groups of patients were all increased,and the dimensions and total scores of the QOL-LC scale in the 4 R model group were higher than those before intervention and general nursing group(P<0.05).Conclusion The application of continuous care based on the 4 R model to patients with advanced liver cancer chemotherapy has a significant effect,which can improve nursing satisfaction,self-care ability and quality of life.
