针刺联合康复技术治疗中风后肩关节半脱位的疗效及其对改善患者临床症状的作用

Analyses on the efficacy of acupuncture combined with rehabilitation technology on shoulder subluxation after stroke and its function on improving patients’clinical symptoms

目的分析针刺联合康复技术治疗中风后肩关节半脱位的疗效及对改善患者临床症状的作用。方法选择粤北人民医院康复医学科在2018年1月至2020年1月收治的90例中风后肩关节半脱位患者,随机数字表法将患者分为对照组、观察组,每组各45例,对照组给予康复技术治疗,观察组联合使用针刺、康复技术治疗,对比两组治疗效果、肩部功能恢复质量、疼痛改善效果。结果对比两组治疗总有效率,观察组为95.56%显著高于对照组的80.00%,差异有统计学意义(P<0.05)。治疗前观察组与对照组AHI、NHISS评分、FMA评分比较,差异无统计学意义(P>0.05)。治疗后两组AHI评分、NHISS评分显著低于治疗前,FMA评分高于治疗前,观察组AHI评分、NHISS评分低于对照组,FMA评分高于对照组,差异有统计学意义(P<0.05)。治疗前观察组与对照组VAS疼痛评分比较,差异无统计学意义(P>0.05)。治疗后7 d、1个月直至治疗结束两组组内VAS评分均低于治疗前,但是观察组VAS疼痛评分显著低于对照组,差异有统计学意义(P<0.05)。结论中风后肩关节半脱位在康复技术治疗的基础上增加针刺治疗,对于整体疗效来讲较为理想,能有效恢复肩关节正常位置,降低疼痛,改善功能,减少神经损伤,有利于提高患者的生活质量,回归正常生活。

Objective To analyze the efficacy of acupuncture combined with rehabilitation technology on shoulder subluxation after stroke and its function on improving patients’clinical symptoms.Methods A total of 90 patients with shoulder subluxation after stroke admitted to the Department of Rehabilitation Medicine of Yuebei People’s Hospital from January 2018 to January 2020 were divided into the control group(n=45)and the observation group(n=45)according to the random number table method.The control group was treated with rehabilitation therapy,while the observation group was treated with acupuncture combined with rehabilitation therapy.The efficacy,shoulder function rehabilitation quality and pain improvement effect in the two groups were compared.Results The total effective rate of treatment in the observation group was 95.56%,which was significantly higher than 80.00%in the control group,with statistically significant difference(P<0.05).Before treatment,there were no statistically significant differences in AHI,NHISS and FMA scores between the observation group and the control group(P>0.05).After treatment,AHI and NHISS scores were significantly lower and FMA scores were higher than those before treatment in each group.Meanwhile,AHI and NHISS scores of the observation group were lower and FMA scores were higher than those of the control group,with statistically significant differences(P<0.05).Before treatment,there was no statistically significant difference in VAS pain scores between the observation group and the control group(P>0.05).7 days after treatment,1 month after treatment and until the end of treatment,the VAS scores in the two groups were both lower than those before treatment.Meanwhile,the VAS pain scores in the observation group were significantly lower than those in the control group,with statistically significant differences(P<0.05).Conclusion Acupuncture treatment for shoulder subluxation after stroke is ideal for the overall efficacy,which can effectively restore the normal position of shoulder joint,reduce the pain,improve the function,and decrease the neurological damage.Therefore,it is conducive to improving the quality of life of patients and returning to normal life.
