不同血凝管对肿瘤患者凝血功能的影响

Impacts of different blood coagulation tubes on coagulation function of tumor patients

目的探讨国产与进口枸橼酸钠血凝管对肿瘤患者凝血功能的影响。方法2019年11—12月分别用国产血凝管(A管)和进口血凝管(B管)采集广西医科大学附属肿瘤医院350例肿瘤患者的血液样本,并统计和比较样本不合格率。选取2019年12月该院62例肿瘤患者(肿瘤患者组)和23名健康人(健康人群组),另外根据所选肿瘤患者是否应用肝素分为肝素组和非肝素组,分别用A管和B管抽取肘部静脉血各一管,同时检测并对比凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(FIB)、抗凝血酶Ⅲ活性(AT-Ⅲ%)、D-二聚体(DD)。结果B管样本不合格率(0.29%)低于A管(5.14%)。各组中两种血凝管TT、FIB、DD结果差异无统计学意义(P>0.05);PT、AT-Ⅲ%结果A管比B管低,差异有统计学意义(P<0.05)。APTT结果只在肝素组中存在差异,差异有统计学意义(P<0.05)。结论进口血凝管标本不合格率低,有利于减少标本采集误差。进口血凝管PT、AT-Ⅲ%结果及经肝素治疗的肿瘤患者的APTT结果可能更加可靠。

Objective To investigate the impacts of domestic and imported sodium citrate blood coagulation tubes on coagulation function of tumor patients.Methods Blood samples of 350 cancer patients admitted to Tumor Hospital Affiliated to Guangxi Medical University from November to December 2019 were collected with domestic blood coagulation tube(tube A)and imported blood coagulation tube(tube B),respectively.And the unqualified rate of samples was counted and compared.Other tumor patients(the tumor patient group,n=62)and healthy people(the healthy people group,n=23)admitted to our hospital in December 2019 were selected.In addition,the patients were divided into the heparin group and the non-heparin group according to whether the selected tumor patients used heparin or not.Venous blood was taken from elbow with tube A and tube B,respectively.At the same time,prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT),fibrinogen(FIB),antithrombinⅢactivity(AT-Ⅲ%)and D-Dimer(DD)were detected and compared.Results The unqualified rate of tube B was 0.29%,which was lower than 5.14%of tube A.There were no statistically significant differences in TT,FIB and DD results between the two blood coagulation tubes in each group(P>0.05).The results of PT and AT-Ⅲ%in tube A were lower than those in tube B(P<0.05).The APTT results were different only in the heparin group(P<0.05).Conclusion The unqualified rate of imported blood coagulation tube specimens is low,which is beneficial to reduce the error of specimen collection.The results of PT and AT-Ⅲ%of imported blood coagulation tubes and APTT results of tumor patients treated with heparin may be more reliable.
