血涂片分析在血常规检验中的应用

Application of Blood Smear Analysis in Blood Routine Test

目的探究血常规检查中应用血涂片检查的临床效果。方法于2017年3月至2019年5月,选取本院就诊进行血常规检查的96例患者作为此次研究的对象,抽取患者4 mL静脉血制备标本两份,分别接受血涂片检查与全自动血细胞分析仪检查,并对不同检查方法的检查结果进行对比。结果血液涂片检查正常标本72例,所占比例75.00%;异常标本24例,所占比例25.00%;全自动细胞分析仪检查正常标本91例,所占比例94.79%;异常标本5例,所占比例5.21%;血液涂片检查正常标本率低于全自动细胞分析仪检查,血液涂片检查异常标本率高于全自动细胞分析仪检查,经分析发现差异有统计学意义(P<0.05)。血涂片检查异常标本中,少量中毒颗粒、不同程度核左移、幼稚粒细胞、核红细胞、红细胞大小不均分别有5例、6例、9例、4例,所占比例分别为20.83%、25.00%、37.50%、16.67%。结论血常规检验中应用血涂片检查可使检验准确率显著提高,使误诊或漏诊率明显降低,为临床治疗提供有利参考,在临床上具有显著的应用价值。

Objective To explore the clinical effect of blood smear examination in routine blood examination.Methods From March 2017 to May 2019,96 patients who received blood routine examination in our hospital were randomly selected as the subjects of this study.All patients and their families voluntarily participated in this study,which was approved by the hospital ethics committee.Four milliliters venous blood was drawn from the patient,and two specimens were prepared.Blood smear examination and automatic blood cell analyzer examination were performed respectively,and the examination results of different examination methods were compared.Results The 72 normal specimens were examined by blood smear,accounting for 75.00%.24 abnormal specimens,accounting for 25.00%;91 normal specimens were examined by automatic cell analyzer,accounting for 94.79%,there were 5 abnormal specimens,accounting for 5.21%.The normal specimen rate of blood smear examination was lower than that of automatic cytometer examination,the abnormal specimen rate of blood smear examination was higher than that of automatic cell analyzer examination,and the difference was statistically significant(P<0.05).In abnormal blood smear specimens,there were 5 cases,6 cases,9 cases and 4 cases of small amount of toxic particles,different degrees of nuclear left shift,immature granulocytes,nuclear red blood cells and uneven red blood cell sizes,accounting for 20.83%,25.00%,37.50%and 16.67%respectively.Conclusion The application of blood smear examination in blood routine examination can significantly improve the accuracy of examination and significantly reduce the rate of misdiagnosis or missed diagnosis,thus providing beneficial reference for clinical treatment and having significant clinical application value.
