补肾化石汤加减结合经皮肾镜碎石术治疗复杂性肾结石42例疗效分析

Treatment of 42 Cases of Complex Kidney Stone with Modified Bushen Huashi Decoction and Percutaneous Nephrolithotomy

目的分析补肾化石汤结合经皮肾镜碎石术应用于复杂性肾结石治疗的临床效果。方法将2014年1月至2019年1月收治的84例较复杂性肾结石患者编入试验组与对照组(每组患者各42例),分别采用补肾化石汤结合经皮肾镜碎石术(试验组)与单纯经皮肾镜碎石术治疗(对照组)。分析两组患者的结石清除率、治疗前后的肾损伤因子指标、生活质量评分、BI指数评分与术后尿源性脓毒血症等并发症发生率。结果试验组的结石清除率高于对照组,并发症发生率低于对照组,差异具有统计学意义(P<0.05)。试验组治疗前的肾损伤因子指标无明显差异(P>0.05),BI指数评分无明显差异(P>0.05),生活质量评分无明显差异(P>0.05)。患者综合治疗后的肾损伤因子指标、BI指数评分及生活质量评分较治疗前均有所改善,组内差异具有统计学意义(P<0.05)。试验组治疗后的肾损伤因子指标、BI指数评分及生活质量评分均优于对照组,组间差异较为显著(P<0.05)。结论补肾化石汤结合经皮肾镜碎石术在复杂性肾结石治疗中的应用,可以有效提升结石清除率,降低术后尿源性脓毒血症等并发症发生率,可以让患者的治疗效果得到明显改善。

Objective To analyze the clinical effect of bushen huashi decoction combined with percutaneous renal lithotripsy in the treatment of complicated renal calculi.Methods Eighty-six patients with complicated renal calculi admitted from January 2014 to January 2019 were enrolled in the experimental group and the control group(42 patients in each group).They were treated with bushen huashi decoction combined with percutaneous renal lithotripsy(experimental group)and simple percutaneous renal lithotripsy(control group).The stone clearance rate,renal injury factor index,quality of life score,BI index score and postoperative urinary sepsis were analyzed.Results The stone removal rate of the experimental group was higher than that of the control group,and the complication rate was lower than that of the control group.The difference was statistically significant(P<0.05).There were no significant differences in the indicators of renal injury factors before treatment in the experimental group(P>0.05),there was no significant difference in the BI index(P>0.05),and there was no significant difference in the quality of life scores(P>0.05).The renal injury factor index,BI index score and quality of life score of the patients after treatment were improved compared with those before treatment.The difference within the group was statistically significant(P<0.05).The renal injury factor index,BI index score and quality of life score of the experimental group were better than the control group,and the difference between the groups was significant(P<0.05).Conclusion The application of bushen huashi decoction combined with percutaneous renal lithotripsy in the treatment of complicated renal calculi can effectively improve the stone clearance rate,reduce the complications such as postoperative urinary sepsis,can significantly improve the patient's treatment effect.
