延续性4C管理模式护理干预对老年高血压患者情绪及生活质量的影响

Effect of continuous 4C management model nursing intervention on emotion and quality of life of elderly patients with hypertension

目的探讨延续性4C管理模式护理干预对老年高血压患者治疗依从性、自我护理能力、情绪及生活质量的影响。方法将151例老年高血压患者按入院时间分为两组,对照组(75例)予以常规护理干预,研究组(76例)在对照组基础上联合延续性4C管理模式护理干预,观察3个月。比较两组治疗依从性、护理满意度、血压控制达标率及干预前后正性负性情绪量表、自我护理能力测定量表、社会功能缺陷筛选量表、杜氏高血压生活质量量表评分。结果研究组治疗总依从率、护理总满意率显著高于对照组(P<0.05或0.01)。干预1个月、3个月末研究组正性负性情绪量表的正性情绪评分显著高于对照组(P<0.01),负性情绪评分低于对照组(P<0.01),自我护理能力测定量表的健康知识水平、自我护理技能、自护责任感、自我概念维度评分及血压控制达标率显著高于对照组(P<0.01),社会功能缺陷筛选量表、杜氏高血压生活质量量表评分显著低于对照组(P<0.01)。结论延续性4C管理模式护理干预能显著改善老年高血压患者治疗依从性、情绪及自我护理能力,提高其血压控制达标率、生活质量及护理满意度。

Objective To explore the effect of continuous 4C management model nursing intervention on treatment compliance,self-care ability,mood and quality of life of elderly patients with hypertension.Methods A total of 151 elderly hypertensive patients were divided into two groups according to the time of admission.The control group(75 cases)received routine nursing intervention,and the study group(76 cases)combined continuous 4C management model nursing intervention on the basis of the control group for 3 months.The treatment compliance,nursing satisfaction and blood pressure control rate of the two groups were compared.The scores of positive and negative emotion scale(PANAS),self-care ability test scale,social disability screening schedule(SDSS)and Du's hypertension quality of life scale of the two groups were compared before and after the intervention.Results The total treatment compliance rate and total nursing satisfaction rate of the study group were significantly higher than those of the control group(P<0.05 or 0.01).At the end of 1 month and 3 months of intervention,the positive emotion score of PANAS in the study group was significantly higher than that in the control group(P<0.01),and the negative emotion score was lower than that in the control group(P<0.01).The health knowledge level,self-care skills,self-care responsibility,self-concept dimension score and blood pressure control compliance rate of the self-care ability test scale were significantly higher than those of the control group(P<0.01).The scores of SDSS and Du's hypertension quality of life scale were significantly lower than those of the control group(P<0.01).Conclusions The continuous 4C management model nursing intervention can significantly improve the treatment compliance,mood and self-care ability of elderly hypertensive patients,and improve their blood pressure control compliance rate,quality of life and nursing satisfaction.
