三级综合医院内科及全科门诊患者焦虑抑郁状况调查

Investigation on anxiety and depression status of outpatients in internal medicine and general outpatient clinic in tertiary general hospitals

目的了解三级综合医院内科及全科门诊患者焦虑抑郁状况,为临床诊治提供依据。方法对三级综合医院内科及全科门诊就诊的1380例患者采用自制资料调查表、焦虑自评量表及抑郁自评量表进行测评分析。结果共1243例患者完成研究,焦虑、抑郁以及焦虑抑郁共病检出率分别为20.0%、22.3%、14.8%。Logistic回归分析显示,年龄、民族、文化程度、就诊科室是焦虑检出率的影响因素(P<0.05或0.01),文化程度、病程是抑郁检出率的影响因素(P<0.05),年龄、民族、文化程度是焦虑抑郁共病的影响因素(P<0.05或0.01)。结论三级综合医院内科及全科门诊患者焦虑、抑郁检出率较高,应对老龄、文化程度较低、疾病早期人群予以焦虑抑郁评估,并及时予以有效干预。

Objective To investigate the anxiety and depression status of outpatients in internal medicine and general outpatient clinic in tertiary general hospitals,and to provide the basis for clinical diagnosis and treatment.Methods A total of 1380 patients in internal medicine and general outpatient clinic of tertiary general hospitals were evaluated and analyzed by using self-made data questionnaire,self-rating anxiety scale(SAS)and self-rating depression scale(SDS).Results A total of 1243 patients completed the study,and the detection rates of anxiety,depression,comorbid anxiety and depression were 20.0%,22.3%and 14.8%respectively.Logistic regression analysis showed that age,nationality,education level and visiting department were the influencing factors of the anxiety detection rate(P<0.05 or 0.01).Education level and course of disease were the influential factors of the depression detection rate(P<0.05),while age,nationality and education level were the influential factors of the comorbid anxiety and depression(P<0.05 or 0.01).Conclusions The detection rate of anxiety and depression in patients in internal medicine and general outpatient clinic of tertiary general hospitals is relatively high.Therefore,anxiety and depression assessment should be carried out in the elderly,those with low educational level and those in the early stage of disease,and timely and effective intervention should be given.
