早期肠内营养对结直肠癌根治术患者胃肠功能恢复的影响 被引量:14

Effect of early enteral nutrition on gastrointestinal function recovery in patients undergoing radical resection of colorectal cancer

摘要 目的探讨早期肠内营养对结直肠癌根治术患者胃肠功能恢复及营养状况的影响。方法选取120例结直肠癌患者,按照随机数字表法分为观察组和对照组,每组60例。观察组在结直肠癌根治术后采取早期肠内营养支持,对照组采取传统营养方法。比较2组患者术后胃肠功能恢复情况、营养状况、不良反应及并发症发生情况。结果观察组患者术后首次排气时间、首次排便时间及住院时间均短于对照组(P<0.05)。2组患者术后不良反应及并发症发生率比较,差异无统计学意义(P>0.05)。2组患者术后2 d血清转铁蛋白(TRF)、血清白蛋白(ALB)、前白蛋白(PA)水平较术前1 d显著降低(P<0.05),但2组间比较差异无统计学意义(P>0.05);2组患者术后8 d血清TRF、ALB、PA水平较术后2 d显著升高(P<0.05),且观察组高于对照组(P<0.05)。结论结直肠癌根治术后早期给予肠内营养能够加快患者胃肠功能恢复,改善营养状况,促进患者康复。 Objective To investigate the effect of early enteral nutrition on gastrointestinal function recovery and nutritional status of patients undergoing radical resection of colorectal cancer.Methods A total of 120 patients with colorectal cancer were selected and they were randomly divided into the observation group and the control group,with 60 cases in each group.Patients in the observation group received early enteral nutrition support after radical resection of colorectal cancer,while patients in the control group adopted traditional nutrition methods.The gastrointestinal function recovery,nutritional status,incidence of adverse reactions and complications after operation in the two groups were compared.Results The first anal exhaust time,the first defecation time and the length of hospital stay after operation in the observation group were shorter than those in the control group(P<0.05).There was no significant difference in the incidence of postoperative adverse reactions or complications between the two groups(P>0.05).The serum levels of transferrin(TRF),albumin(ALB),and prealbumin(PA)2 days after operation in the two groups were significantly lower than those 1 day before operation(P<0.05),but no significant difference was found between the two groups(P>0.05);the serum levels of TRF,ALB,and PA 8 days after operation in the two groups were significantly higher than those 2 days after operation(P<0.05),and those in the observation group were higher than those in the control group(P<0.05).Conclusion Early enteral nutrition after radical resection of colorectal cancer can accelerate the recovery of gastrointestinal function,improve the nutritional status and promote the rehabilitation of patients.
