消脂护肝汤联合水飞蓟宾对非酒精性脂肪肝患者糖脂代谢水平及肝功能的影响

Effect of Xiaozhi Hugan Decoction Combined With Silibinin on the Level of Glucose and Lipid Metabolism and Liver Function in Patients With Non-alcoholic Fatty Liver

目的研究消脂护肝汤联合水飞蓟宾对非酒精性脂肪肝患者糖脂代谢水平及肝功能的影响。方法选取108例非酒精性脂肪肝患者为研究样本,纳入时间段为2018年10月—2020年10月,分组方法为随机数字表法,分为两组,每组54例。对照组采用水飞蓟宾治疗,观察组在对照组治疗的基础上加用消脂护肝汤治疗。比较两组患者治疗前后胁肋胀痛、腹胀、食少纳呆3项症状积分,分析两组治疗前后糖脂代谢水平[甘油三脂(Triglyceride,TG)、总胆固醇(Total cholesterol,TC)、高密度脂蛋白胆固醇(High density lipoprotein cholesterol,HDL-C)、低密度脂蛋白胆固醇(Low density lipoprotein cholesterol,LDL-C)、空腹血糖(Fasting blood glucose,FBG)、葡萄糖代谢率(Glucose metabolic rate,M)]及肝功能指标[丙氨酸转氨酶(Alanine aminotransferase,ALT)、天冬氨酸转氨酶(Aspartate aminotransferase,AST)、γ-谷氨酰转肽酶(Gamma glutamyl transpeptidase,GGT)],并观察两组患者不良反应发生情况。结果两组患者治疗后胁肋胀痛、腹胀、食少纳呆症状积分均明显低于本组治疗前,差异有统计学意义(P<0.05),观察组治疗后胁肋胀痛、腹胀、食少纳呆症状积分均明显低于对照组,差异有统计学意义(P<0.05)。观察组治疗后TG、TC、LDL-C水平均明显低于本组治疗前及对照组治疗后,差异有统计学意义(P<0.05),HDL-C、M水平明显高于本组治疗前及对照组治疗后,差异有统计学意义(P<0.05)。观察组治疗后GGT、ALT、AST水平均明显低于本组治疗前及对照组治疗后,差异有统计学意义(P<0.05)。两组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论消脂护肝汤联合水飞蓟宾能够对改善非酒精性脂肪肝患者糖脂代谢水平,保护肝功能。

Objective To study the effect of Xiaozhi Hugan Decoction Combined with silibinin on the level of glucose and lipid metabolism and liver function in patients with nonalcoholic fatty liver disease.Methods 108 cases of patients with nonalcoholic fatty liver disease in our hospital were selected as the research sample.The included time period was from October 2018 to October 2020.They were randomly divided into two groups,54 cases in each group.The control group was treated with silibinin,and the observation group was treated with Xiaozhi Hugan Decoction on the basis of the control group.The symptom scores of hypochondriac distension,abdominal distension and anorexia were compared between the two groups before and after treatment.The levels of glucose and lipid metabolism[triglyceride(TG),total cholesterol(TC),High density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),fasting blood glucose(FBG),glucose metabolic rate(M)]and liver function indexes[alanine aminotransferase(ALT),aspartate aminotransferase(AST),gamma glutamyl transpeptidase(GGT)]were analyzed before and after treatment,the adverse reactions of the two groups were observed.Results After treatment,the scores of flank pain,abdominal distension,and low appetite and anorexia symptoms in the two groups were significantly lower than those before treatment(P<0.05).After treatment in the observation group,the scores of flank pain,abdominal distension,and low appetite and anorexia symptoms were all significantly lower than the control group(P<0.05).After treatment,the levels of TG,TC,and LDL-C in the observation group were significantly lower than before and after treatment in the control group(P<0.05),and HDL-C and M were significantly higher than before and after treatment in the control group(P<0.05).The levels of GGT,ALT,and AST in the observation group after treatment were significantly lower than those before and after treatment in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Xiaozhi Hugan Decoction Combined with silibinin has important clinical value in improving glucose and lipid metabolism and protecting liver function in patients with nonalcoholic fatty liver.
