动态血压监测慢性肾脏病合并高血压患者在夜间高血压的患病率及影响因素 被引量:2

摘要 目的分析动态血压监测慢性肾脏病合并高血压患者在夜间高血压的患病率及影响因素。方法选择我院在2019年1月至2020年6月期间收治的100例慢性肾脏病合并高血压患者,对其临床资料进行回顾性分析,根据其动态血压监测情况以有无发生夜间高血压作为分组方法,将其分为A组(发生夜间高血压,n=90)和B组(没有发生夜间高血压,n=10),对影响其夜间高血压发生的因素行单因素及多因素分析。结果两组患者在肾小球滤过率、慢性肾脏病分期、高血压分级、血红蛋白、白蛋白、尿酸氮、肌酐、尿酸、同型半胱氨酸以及甲状旁腺素的比较中,差异形成统计学意义(P<0.05)。多因素分析结果显示夜间高血压发生的独立危险因素为肾小球滤过率、白蛋白、肌酐以及同型半胱氨酸(P<0.05)。结论对于慢性肾脏病合并高血压患者群体,具有较高的夜间高血压患病率,导致其发生夜间高血压的主要独立危险因素为肾小球滤过率、白蛋白、肌酐以及同型半胱氨酸。 Objective To analyze the prevalence and influencing factors of night-time hypertension in patients with chronic kidney disease and hypertension in ambulatory blood pressure monitoring. Methods A total of 100 patients with chronic kidney disease and hypertension admitted to our hospital between January 2019 and June 2020 were selected, and their clinical data were retrospectively analyzed. According to their ambulatory blood pressure monitoring, the presence of nocturnal hypertension was taken as Grouping method, divide them into group A(nocturnal hypertension, n=90) and group B(no nocturnal hypertension, n=10). Single-factor and multi-factor analysis of factors affecting the occurrence of nocturnal hypertension. Results The differences between the two groups of patients in the comparison of glomerular filtration rate, chronic kidney disease stage, hypertension grade, hemoglobin, albumin, nitrogen urate, creatinine, uric acid, homocysteine and parathyroid hormone, formed statistics Academic significance(P<0.05). The results of multivariate analysis showed that the independent risk factors for nocturnal hypertension were glomerular filtration rate, albumin, creatinine and homocysteine(P<0.05). Conclusion For patients with chronic kidney disease and hypertension, there is a high prevalence of nocturnal hypertension. The main independent risk factors leading to nocturnal hypertension are glomerular filtration rate, albumin, creatinine and homocysteamine acid.
