阿立哌唑联合碳酸锂治疗双相情感障碍患者的效果及对BRMS评分的影响

Effect of Aripiprazole Combined with Lithium Carbonate in the Treatment of Bipolar Disorder and Its Influence on BRMS Score

目的分析阿立哌唑联合碳酸锂对双相情感障碍患者的效果。方法前瞻性选取我院2018年1月~2020年8月期间接收的双相情感障碍患者104例,采用电脑随机数字表法以1∶1比例分为2组,各52例。对照组接受碳酸锂治疗,研究组在对照组基础上加用阿立哌唑治疗,比较两组临床疗效、不良反应状况及治疗前、治疗1周、8周阳性和阴性症状量表(positive and negative syndrome scale,PANSS)、治疗前后抑郁自评量表(self-rating depression scale,SDS)、贝克-拉范森躁狂量表(BechRafaelsdn mania rating scale,BRMS)、血清脑源性神经营养因子(brain derived neurotrophic factor,BDNF)、同型半胱氨酸(homocysteine,Hcy)水平。结果研究组总有效率96.15%(50/52)高于对照组80.77%(42/52,P<0.05);治疗1周、8周研究组PANSS评分低于对照组(P<0.05);治疗后两组SDS、BRMS评分低于治疗前,研究组低于对照组(P<0.05);治疗前后两组血清Hcy、BDNF水平对比,差异具有统计学意义,且研究组优于对照组(P<0.05);与对照组26.92%(14/52)对比,研究组不良反应发生率5.76%(3/52)明显降低(P<0.05)。结论阿立哌唑联合碳酸锂治疗双相情感障碍患者效果显著,可改善临床症状,减轻抑郁、狂躁情绪,调节血清Hcy、BDNF水平,降低不良反应发生率。

OBJECTIVE To analyze the effect of aripiprazole combined with lithium carbonate on patients with bipolar disorder.METHODS 104 patients with bipolar disorder in our hospital from January 2018 to August 2020 were prospectively selected and randomly divided into two groups with 52 cases in each group.The control group was treated with lithium carbonate,while the study group was treated with aripiprazole on the basis of the control group.The clinical efficacy,adverse reactions,positive and negative syndrome scale(PANSS)and self rating depression scale(SDS)before and after treatment were compared between the two groups.The serum levels of brain-derived neurotrophic factor(BDNF)and homocysteine(Hcy)were measured with the Beck Rafaelsdn mania rating scale(BRMS).RESULTS The total effective rate of the study group was 96.15%(50/52),which was higher than 80.77%(42/52)of the control group(P<0.05).The PANSS score of the study group was lower than that of the control group after 1 week and 8 weeks of treatment(P<0.05).The SDS and BRMS scores of the two groups after treatment were lower than those before treatment,and the study group was lower than that of the control group(P<0.05).The serum Hcy and BDNF levels of the two groups before and after treatment had statistical significance,and the study group was better than the control group.Compared with 26.92%(14/52)in the control group,the incidence of adverse reactions in the study group was 5.76%(3/52),which was significantly lower(P<0.05).CONCLUSION Aripiprazole combined with lithium carbonate is effective in the treatment of bipolar disorder,which can improve clinical symptoms,relieve depression and manic mood,regulate serum Hcy and BDNF levels,and reduce the incidence of adverse reactions.
