胸腔镜下肺癌根治术治疗老年非小细胞肺癌的疗效观察

Efficacy of thoracoscopic radical resection of lung cancer in elderly patients with non-small cell lung cancer

目的探讨胸腔镜下肺癌根治术治疗老年非小细胞肺癌的临床疗效。方法选取2019年1月至2020年1月间杨凌示范区医院收治的106例老年非小细胞肺癌患者,采用随机数表法分为观察组和对照组,每组53例。观察组患者行胸腔镜下肺癌根治术,对照组患者行开胸手术治疗,比较两组患者术中出血量、手术时间、淋巴结组数、术后带管时间、术后引流量、并发症及生活质量。结果两组患者淋巴结组数和手术时间比较,差异无统计学意义(P>0.05)。观察组患者术中出血量和术后引流量均低于对照组,术后带管时间短于对照组,差异均有统计学意义(均P<0.05)。观察组患者并发症发生率为3.8%,低于对照组的15.1%,差异有统计学意义(P<0.05)。术前及术后6个月,两组患者生活质量评分比较,差异无统计学意义(P>0.05)。术后1个月及3个月,观察组患者生活质量评分均高于对照组,差异均有统计学意义(均P<0.05)。结论胸腔镜下肺癌根治术治疗老年非小细胞肺癌疗效较好,创伤性小,术后恢复快,有利于改善患者术后生存情况。

Objective To investigate the efficacy of thoracoscopic radical resection for the treatment of elderly patients with non-small cell lung cancer.Methods A total of 106 elderly patients with nonsmall cell lung cancer who were treated at Yangling Demonstration Area Hospital from January 2019 to January 2020 were selected.Using the random number table method,they were divided into an observation group and a control group with 53 patients in each group.The observation group underwent thoracoscopic radical resection of lung cancer and the control group thoracotomy.The intraoperative blood loss,duration of operation,number of lymph node groups,duration of postoperative catheterization,postoperative drainage,complications and quality of life were compared between the two groups.Results There was no statistically significant difference in the number of lymph node groups and uduration of operation between the two groups(P>0.05).Intraoperative blood loss and postoperative drainage volume were lower in the observation group than in the control group,and the duration of postoperative catheterization was shorter in the observation group than in the control group(all P<0.05).The incidence of complications was 3.8%in the observation group which was lower than 15.1%of the contro group(P<0.05).Before and 6 months after the operation,quality of life scores were not statistically different between the two groups(P>0.05).At 1 month and 3 months after the operation,quality of life scores were higher in the observation group than in the control group(all P<0.05).Conclusion Thoracoscope radical resection of lung cancer is effective for the treatment of elderly patients with non-small cell lung cancer,with less trauma and faster postoperative recovery.At the same time,it contributes to improving the survival of patients after surgery.
