风险分级管理预防下肢深静脉血栓导管接触性溶栓出血的效果分析

Effect of Risk Grading Management on Prevention of Contact Thrombolytic Hemorrhage in Deep Vein Thrombosis of Lower Extremities

目的:探讨风险分级管理预防导管接触性溶栓患者出血的效果。方法:选择2019年8月至2020年4月收治的80例接受导管接触性溶栓治疗的下肢深静脉血栓形成患者,随机分为观察组和对照组各40例,对照组患者溶栓期间接受常规护理,观察组患者接受风险分级管理方案,比较2组溶栓治疗效果和出血事件发生率。结果:2组平均溶栓时间、平均尿激酶用量、静脉通畅率比较差异无统计学意义(P>0.05);治疗后观察组MAADP、PRI明显高于对照组(P<0.05);2组患者溶栓期间均未发生严重出血事件,观察组轻微出血事件发生率10.00%明显低于对照组32.50%(P<0.05)。结论:风险分级管理在导管接触性溶栓患者中应用良好,可以减少出血事件的发生,提高溶栓治疗安全性。

[Objective]To investigate the effect of risk grading management on the prevention of bleeding in patients with catheter contact thrombolysis.[Method]A total of 80 patients with catheter contact thrombolysis were randomly divided into observation group(40 cases)and control group(40 cases).Patients in the control group received routine nursing during thrombolysis,while patients in the observation group received risk grading management.The effect and incidence of bleeding events were compared between the two groups.[Result]There were no statistically significant differences in average thrombolysis time,average amount of urokinase and venous patency rate between the two groups(P>0.05).After treatment,MAADP and PRI in observation group were significantly higher than those in control group(P<0.05).The incidence of minor bleeding events in the observation group(10.00%)was significantly lower than that in the control group(32.50%,P<0.05).[Conclusion]Risk grading management is a good application in patients with catheter contact thrombolysis,which can reduce the occurrence of bleeding events and improve the safety of thrombolytic therapy.
