慢性乙型肝炎合并不孕症患者外周血基质细胞衍生因子1、趋化因子受体4水平与子宫内膜容受性的相关性

Correlation of peripheral blood stromal cell-derived factor 1 and CXC motif chemokine receptor 4 levels with endometrial receptivity in infertile patients complicated with chronic hepatitis B

目的探讨慢性乙型肝炎(CHB)合并不孕症患者外周血基质细胞衍生因子1(SDF-1)、CXC基序趋化因子受体4(CXCR4)水平与子宫内膜容受性的相关性。方法根据是否合并不孕症将112例CHB患者分为CHB合并不孕组(30例)和CHB组(82例),另选取同期体检的70例健康女性作为对照组。比较3组研究对象的血清SDF-1、CXCR4水平、子宫内膜螺旋动脉搏动指数、阻力指数及子宫内膜厚度(EMT)。采用Pearson相关分析评估CHB合并不孕症患者血清SDF-1、CXCR4水平与子宫内膜容受性指标的相关性,采用多因素Logistic回归模型分析CHB患者合并不孕症的影响因素。结果对照组、CHB组及CHB合并不孕组的外周血SDF-1、CXCR4水平以及子宫内膜容受性指标子宫内膜螺旋动脉搏动指数、子宫内膜螺旋动脉阻力指数依次升高,EMT依次降低(均P<0.05)。Pearson相关分析结果显示,CHB合并不孕症患者血清SDF-1、CXCR4水平与子宫内膜螺旋动脉搏动指数、子宫内膜螺旋动脉阻力指数水平均呈正相关,与EMT均呈负相关关系(均P<0.05)。多因素Logistic回归分析结果显示,SDF-1、CXCR4、子宫内膜螺旋动脉搏动指数、子宫内膜螺旋动脉阻力指数及EMT均是CHB患者合并不孕症的影响因素(均P<0.05)。结论CHB合并不孕症患者血清SDF-1、CXCR4水平均升高,且与患者子宫内膜容受性指标子宫内膜螺旋动脉搏动指数、子宫内膜螺旋动脉阻力指数、EMT关系密切,均是CHB患者合并不孕症的影响因素。

Objective To explore the correlation of peripheral blood stromal cell-derived factor 1(SDF-1)and CXC motif chemokine receptor 4(CXCR4)levels with endometrial receptivity in infertile patients complicated with chronic hepatitis B(CHB).Methods A total of 112 CHB patients were divided into infertile CHB group(30 cases)and CHB group(82 cases)according to the presence of concomitant infertility,concurrently,another 70 healthy women who undertook physical examination were selected as control group.The related indicators were compared among the three groups,including serum SDF-1 and CXCR4 levels,endometrial spiral artery pulsatility index and resistance index,and endometrial thickness(EMT).Pearson correlation analysis was applied to assessing the correlation between serum SDF-1 and CXCR4 levels and endometrial receptivity indicators among infertile patients with CHB,and the multivariate Logistic regression model was employed to analyze the influencing factors of concomitant infertility in CHB patients.Results Peripheral blood SDF-1 and CXCR4 levels and endometrial receptivity indicators endometrial spiral artery pulsatility index and endometrial spiral artery resistance index increased,whereas EMT decreased in the sequence of the control group,the CHB group and the infertile CHB group(all P<0.05).The results of Pearson correlation analysis showed that,in infertile patients with CHB,serum SDF-1 and CXCR4 levels positively correlated with the levels of endometrial spiral artery pulsatility index and endometrial spiral artery resistance index,and negatively correlated with EMT(all P<0.05).Multivariate Logistic regression analysis results showed that SDF-1,CXCR4,endometrial spiral artery pulsatility index,endometrial spiral artery resistance index,and EMT were the influencing factors of concomitant infertility in CHB patients(all P<0.05).Conclusion Serum SDF-1 and CXCR4 levels are increased in infertile patients with CHB,and are closely related to endometrial receptivity indicators endometrial spiral artery pulsatility index,endometrial spiral artery resistance index and EMT,which are the influencing factors of concomitant infertility in CHB patients.
