超声心动图检查在诊断新生儿持续性肺动脉高压中的应用价值 被引量:1

摘要 目的:探讨超声心动图检查在诊断新生儿持续性肺动脉高压(PPHN)中的应用价值。方法:将常州市妇幼保健院收治的60例PPHN患儿和在该院出生的40例健康新生儿作为研究对象。将其中40例健康新生儿作为对照组,将其中60例PPHN患儿作为观察组。对两组新生儿均进行超声心动图检查。然后,比较两组新生儿出生的时间、胎龄、体质量、收缩压(SBP)、舒张压(DBP)、左心室射血分数(LVEF)、每搏输出量(SV)及其进行超声心动图检查的结果。结果:两组新生儿出生的时间、胎龄、体质量、SBP、DBP、LVEF及SV相比,P>0.05。与对照组新生儿相比,观察组患儿的三尖瓣环收缩峰值位移(TAPSE)较小,其三尖瓣环收缩峰值速度(TAPSV)较慢,其右心室面积变化分数(RVFAC)较低,P<0.05。观察组患儿中生存患儿与死亡患儿的主肺动脉内径(MPAD)及右心室舒张末期内径(RVDd)相比,P>0.05。与观察组患儿中死亡的患儿相比,观察组患儿中生存患儿的左心室舒张末期内径(LVDd)及TAPSE均较大,其TAPSV较快,其RVDd/LVDd较小,其RVFAC较高,P<0.05。结论:超声心动图检查在诊断新生儿PPHN中的应用价值显著。 Objective:To investigate the value of echocardiography in the diagnosis of neonatal persistent pulmonary hypertension(PPHN).Methods:60 children with PPHN admitted to Changzhou Maternal and Child Health Hospital and 40 healthy newborns born in the hospital were selected as research objects.Among them,40 healthy newborns were used as control group and 60 PPHN children were used as observation group.Echocardiography was performed in both groups.Then,time of birth,gestational age,body weight,systolic blood pressure(SBP),diastolic blood pressure(DBP),left ventricular ejection fraction(LVEF),stroke output(SV)and echocardiographic results were compared between the two groups.Results:The birth time,gestational age,body weight,SBP,DBP,LVEF and SV of the two groups were compared,P>0.05.Compared with the control group,the observation group had smaller tricuspid ring peak contraction displacement(TAPSE),slower tricuspid ring peak contraction velocity(TAPSV),and lower right ventricular area change fraction(RVFAC)(P<0.05).The main pulmonary artery diameter(MPAD)and right ventricular end diastolic diameter(RVDd)of the surviving children in the observation group were compared with those of the dead children,P>0.05.Compared with the children who died in the observation group,the survival children in the observation group had larger LVDd and TAPSE,faster TAPSV,smaller RVDd/LVDd,and higher RVFAC(P<0.05).Conclusion:Echocardiography has a significant value in the diagnosis of neonatal PPHN.
