新时代中国西部地区基层医疗机构医生工作满意度分析及其良性发展策略

Analysis of Doctors’Job Satisfaction in Primary Medical Institutions in Western China in the New Era and Their Benign Development Strategy

摘要 目的分析中国西部基层医生工作满意度现状及其影响因素,为政府制定基层医生的良性发展策略提供理论依据。方法使用2011年6—12月中国西部11个省横断面调查获得调查者资料,使用自行设计的问卷收集研究对象的信息,应用分位数回归(QR)和多水平模型(MLM)分析工作满意度的影响因素。结果基层医生总体满意度偏低,在不同工作满意度得分分位数中,QR的结果与MLM并不完全一致。整体而言,年龄≥46岁,收入满意度高,离职意向低,单位政策认同高,个人规划好,职业态度好,工作价值高且患者认可度高将带来更高的工作满意度。结论建议政府完善激励机制,提升自主权和灵活性,同时提高单位政策认同力度,建立更多的非财务激励措施,增加继续教育机会,改善他们的职业规划,提高基层医生的职业认同。 Objective To analyze the current status of job satisfaction of grassroots doctors in western China and its influencing factors,and provide a theoretical basis for the government to formulate a benign development strategy for grassroots doctors.Methods A cross-sectional survey of 11 provinces in western China from June to December 2011 was used to obtain investigator data,a self-designed questionnaire was used to collect information on the research subjects,and quantile regression(QR)and multilevel model(MLM)were applied to analyze the influencing factors of job satisfaction.Results The overall satisfaction of primary-level doctors was low.Among the different quantiles of job satisfaction scores,the results of QR and MLM were not completely consistent.On the whole,age≥46,high income satisfaction,low turnover intention,high unit policy approval,good personal planning,good professional attitude,high job value and high patient recognition will bring higher job satisfaction.Conclusion It is recommended that the government improve the incentive mechanism,increase autonomy and flexibility,and at the same time increase unit policy recognition,establish more non-financial incentives,increase continuing education opportunities,improve their career planning,and increase the professional recognition of grassroots doctors.
