封闭式吸痰联合全胸振荡排痰机对危重症患者排痰效果与呼吸功能影响的观察

Observation of effects of closed sputum aspiration combined with whole-chest oscillatory sputum excretion machine on sputum excretion and respiratory function in critically ill patients

目的:探究封闭式吸痰联合全胸振荡排痰机对危重症患者排痰效果与呼吸功能的影响。方法:选择2019年5月~2020年5月于急诊内科治疗的90例行机械通气的患者,随机分为观察组与对照组各45例。对照组患者的呼吸道护理采用人工叩背排痰法与常规开放式吸痰,观察组患者予以封闭式吸痰联合全胸振荡排痰机护理。比较两组患者干预前后二氧化碳分压及动脉氧氧合指数等呼吸功能指标,以及每日总吸痰的时间、次数与总排痰量。比较两组患者ICU住院时间与机械通气时间,并对患者家属进行护理满意度调查。结果:干预后观察组患者每日吸痰时间、吸痰次数与每日平均排痰量均低于对照组患者,且二氧化碳分压明显降低,动脉氧氧合指数显著增高;观察组患者ICU住院时间与机械通气时间均明显低于对照组患者,且患者家属护理满意度显著增高,以上差异均有统计学意义(P<0.05)。结论:封闭式吸痰联合全胸振荡排痰机有助于提高危重症患者的排痰效果,并改善患者的呼吸功能。

Objective To explore the influence of closed sputum aspiration combined with whole-chest oscillatory sputum excretion machine on the sputum excretion effect and respiratory function in critically ill patients.Method 90 patients undergoing mechanical ventilation during emergency medical treatment from May 2019 to May 2020 were randomly divided into observation group and control group,with 45 patients in each group.The respiratory tract nursing of the patients in the observation group was performed by manual tapping and sputum excretion method and conventional open sputum aspiration,while the patients in the control group were treated with closed sputum aspiration combined with whole-chest oscillation sputum excretion machine.The respiratory function indicators such as carbon dioxide partial pressure and arterial oxygenation index before and after intervention were compared between the two groups,as well as the time,frequency and total sputum excreted per day.The length of ICU stay and mechanical ventilation time were compared between the two groups,and nursing satisfaction of the patients′family members was investigated.Results After intervention,the sputum aspiration time,sputum aspiration times and average sputum excretion per day in the observation group were all lower than those in the control group,and the partial pressure of carbon dioxide was significantly reduced,and the arterial oxygenation index was significantly increased.The length of ICU stay and mechanical ventilation time of patients in the observation group were significantly lower than those in the control group,and the nursing satisfaction of patients′family members was significantly increased(P<0.05).Conclusion Enclosed sputum aspiration combined with whole-chest oscillatory sputum excretion machine is helpful to improve the sputum excretion effect and respiratory function in critically ill patients.
