Effects of duodenal diverticulum on biliary tract diseases and ERCP intubation

目的回顾性探究十二指肠乳头旁憩室对胆道疾病与ERCP插管的影响,以及探究胆总管结石复发的危险因素。方法对我院行ERCP取石的970例胆石症患者的临床资料进行回顾性分析。按有无PAD分为PAD组和非PAD组,探究PAD对原发性胆总管结石胆管炎、胆总管直径、结石的个数和大小、及ERCP插管成功率的影响;按有无结石复发把原发性胆总管结石分为复发和未复发两组,探究结石复发的独立危险因素。结果单因素分析显示PAD组原发性胆总管结石、胆管炎、结石直径>1.0cm与无PAD组差异有统计学意义;Logistie多元回归分析发现,胆管炎胆总管直径>1.5cm胆道手术、单次住院行ERCP次数≥2次为结石复发的独立危险因素。结论PAD与原发性胆总管结石、胆管炎、结石直径>1.0cm有密切联系;胆管炎、胆总管直径>1.5cm、胆道手术、单次住院行ERCP次数≥2次为结石复发的独立危险因素。

Objective A retrospective study was conducted to investigate the ffeet of PAD on the biliary tract disease and ERCP intubation,and risk factors for recurrence of common bile duct stones.Methods The clinical data of 970 patients with choleli-thiasis who underwent ERCP lithotomy in our hospital were retrospectively analyzed.According to whether there is a PAD,divides into PAD group and non-PAD group,To explore the influence of PAD on primary common bile duct stones,cholangitis,common bile duct diameter,number and size of stones,and success rate of ERCP intubation.According to the presence of recurrence,divides the prima-ry calculi into recurrence and no recurrence groups,to identify the independent risk factors of recurrence.Results Single analysis showed that there were statistically significant differences in primary choledocholithiasis,cholangitis,and stone diameter>1.0cm be-tween the PAD group and the non-PAD group.Logistic multiple regression analysis found that cholangitis,biliary duct diameter that is greater than 1.5cm,biliary surgery,and ERCP times of single hospitalization≥2 times were independent risk factors for stone recur-rence.Conclusion PAD was closely associated with primary choledocholithiasis,cholangitis,and the diameter of stones>1.0cm;cholangitis,biliary duct diameter that is greater than 1.5em,biliary surgery,and ERCP times of single hospitalization≥2times were in-dependent risk factors for stone recurrence.
