摘要
目的探究开滦研究人群动脉硬化发病情况以及新发动脉硬化的影响因素。方法选取参加第2次随访及后续任意一次随访(截止到2019年11月31日),并完成2次臂踝动脉脉搏波传导速度(b aPWV)检测,且既往无心房颤动及动脉硬化的在职及退休职工9958人作为观察对象。采用多因素logistic回归分析探究新发动脉硬化的影响因素。结果在平均随访3.8年后,新发动脉硬化177例,动脉硬化发病密度为4.65/千人年,标化发病密度为7.33/千人年。多因素logistic回归分析结果显示,增龄(OR 1.095,95%CI 1.078~1.113)、基线心率增加(OR 1.027,95%CI 1.016~1.038)、高血压(OR 2.548,95%CI 1.839~3.530)、糖尿病(OR 2.533,95%CI 1.661~3.864)及血脂异常(OR 1.526,95%CI 1.111~2.097)是新发动脉硬化的危险因素(均P<0.05)。结论男性动脉硬化发病密度高于女性。影响动脉硬化发病的主要危险因素是高血压、糖尿病、血脂异常。
Objective To explore the incidence of arterial stiffness and the risk factors of new arterial stiffness in Kailuan cohort.Methods A total of 9958 participants of Kailuan cohort without atrial fibrillation and arterial stiffness were included in this study.Brachial-ankle pulse wave velocity(baPWV)was tested at least twice during the study(with deadline of November 31,2019).Multivariate logistic regression analysis was used to analyze risk factors of new-onset arterial stiffness.Results After a median of 3.8 years follow-up,there were 177 new-onset arterial stiffness participants.The incidence density of arterial stiffness was 4.65/1000 person-years,and the standardized incidence density was 7.33/1000 person-years.Multivariate logistic regression analysis showed that age(OR 1.095,95%CI 1.078-1.113),higher heart rate(OR 1.027,95%CI 1.016-1.038)at baseline,as well as hypertension(OR 2.548,95%CI 1.839-3.530),diabetes(OR 2.533,95%CI 1.661-3.864),and dyslipi-demia(OR 1.526,95%CI 1.111-2.097)were risk factors for new-onset arterial stiffness.Conclusions The in cidence of arteriosclerosis in men is higher than women.The main risk factors of arteriosclerosis are hypertension»diabetes,and dyslipidemia.
