补阳还五汤联合针灸对脑卒中后遗症患者的治疗疗效观察

Clinical effects of the Buyang Huanwu decoction plus acupuncture on stroke sequelae

目的:探讨补阳还五汤联合针灸对脑卒中后遗症患者的治疗效果。方法:在2017年9月-2019年4月期间,纳入安阳市脉管炎医院诊治的110例脑卒中后遗症患者为研究对象,并按随机抽样法予以分组:对照组(55例)采取常规西药治疗,研究组(55例)则在常规西药治疗基础上加用补阳还五汤联合针灸疗法,均坚持治疗6个月。运用统计学方法,比较两组患者的脑卒中后遗症总体疗效,同时参考美国国立卫生研究院卒中量表(National Institates of Health Stroke Scale,NIHSS)观察患者治疗前后的神经功能缺损程度、中医证候积分以及生活质量评分变化。结果:研究组患者的脑卒中后遗症总体有效率高于对照组(χ2=7.040,P=0.008);两组治疗后的NIHSS评分、中医证候积分、健康问卷躯体症状群量表(PHQ-15)评分均低于治疗前,并以研究组中上述三项评分更低(NIHSS评分:t=20.332、14.542、12.228,均P<0.001;中医证候积分:t=64.453、38.780、18.477,均P<0.001;PHQ-15量表评分:t=12.007、16.471、13.519,均P<0.001)。研究组生活自理独立率高于对照组(χ2=7.745,P<0.05)。结论:补阳还五汤联合针灸对脑卒中后遗症患者的治疗效果突出,可有效改善患者的中医证候,最大限度地恢复神经功能,提高其出院后的生活质量水平,可借鉴。

Objective:To explore the therapeutic effects of the Buyang Huanwu decoction(补阳还五汤)plus acupuncture on stroke sequelae.Methods:110 cases from September 2017 to April 2019 were included as the research objects and were grouped according to the random sampling method:the control group(55 cases)was treated with conventional western medicine,the study group(55 cases)was treated with conventional western medicine plus the Buyang Huanwu decoction and acupuncture,and the treatment lasted for 6 months.Results:The total efficiency of stroke sequelae in the study group was higher than that in the control group(χ2=7.040,P=0.008);the NIHSS scores,TCM syndrome scores,and PHQ-15 scores in the groups were lower than before treatment,and those indicators in the study group were lower than the control group(NIHSS score:t=20.332,14.542,12.228,all P<0.001;TCM syndrome score:t=64.453,38.780,18.477,both P<0.001;PHQ-15 scale score:t=12.007,16.471,13.519,all P<0.001).The independent rate of self-care in the study group was higher than the control group(χ2=7.745,P<0.05).Conclusion:The Buyang Huanwu decoction plus acupuncture shows outstanding effects on stroke sequelae,which can effectively ameliorate TCM syndromes of patients,maximize the recovery of nerve function,and improve life quality of patients after discharge,which can be used for reference.
