腹腔镜手术治疗老年腹股沟疝的效果及安全性分析

Effect and Safety Analysis of Laparoscopic Surgery in the Treatment of Elderly Inguinal Hernia

目的 比较老年腹股沟疝患者采取传统开腹和腹腔镜手术治疗的效果,探讨腹腔镜手术治疗老年腹股沟疝的应用价值.方法 选取2018年1月—2020年1月沈阳医学院附属第二医院收治的76例老年腹股沟疝患者,所有患者依据手术方案的不同分为观察组和对照组各38例.对照组采取传统开放补片修补术治疗,观察组采用腹腔镜手术治疗.观察并比较两组患者围手术期相关指标,以及并发症和疾病复发情况.结果 观察组手术时间、住院时间,以及术后胃肠道恢复正常时间和自主活动时间均短于对照组,术中出血量少于对照组,两组上述指标差异均具有统计学意义(P均<0.05).观察组总并发症发生率低于对照组,差异具有统计学意义(P<0.05).两组疾病复发率差异无统计学意义(P>0.05).结论 采用腹腔镜治疗老年腹股沟疝的疗效确切,且该术式具有创伤小、患者术后恢复快、并发症少等优势,值得临床推广应用.

Objective To compare the effects of traditional laparotomy and laparoscopic surgery in elderly patients with inguinal hernia,and to explore the application value of laparoscopic surgery in the treatment of elderly inguinal hernias.Methods A total of 76 elderly patients with inguinal hernia admitted to the Second Affiliated Hospital of Shenyang Medical College from January 2018 to January 2020 were selected,and divided into control group and observation group according to different treatment schemes,with 38 cases in each group.The control group was treated with traditional open mesh repair,and the observation group was treated with laparoscopic surgery.The perioperative indicators,complications and disease recurrence of the two groups were observed and compared.Results The operation time,hospitalization time,postoperative gastrointestinal recovery time and autonomous activity time in observation group were significantly shorter than those of the control group,and the intraoperative blood loss was significantly less than that of the control group(P<0.05).The incidence rate of complications in the observation group was significantly lower than that of the control group(P<0.05).There was no significant difference of recurrence rate between the two groups(P>0.05).Conclusion Laparoscopic surgery in the treatment of elderly inguinal hernia has the advantages of short operation time,less trauma,fewer complications and exact curative effect,which is suitable for clinical application.
