中药“止痒散”外洗剂治疗特应性皮炎的临床疗效观察

Clinical observation of the curative effect of traditional Chinese medicine Zhiyang powder external lotion on atopic dermatitis

目的:探讨中药止痒散外洗剂治疗特应性皮炎(Atopic Dermatitis,AD)的临床疗效,评估患者生活质量,为临床治疗提供依据。方法:选择普宁市人民医院已确诊的62例特应性皮炎患者为观察对象,依照随机数字表法分为观察组(31例)与对照组(31例)。对照组给予患者外擦丙酸氟替卡松治疗,观察组予以中药止痒散外洗剂治疗。观察两组治疗后皮损、瘙痒情况,采用特应性皮炎评分系统(Scoring Atopic Dermatitis,SCORAD)评估两组疗效,应用儿童皮肤病生活质量指数(The Children's DermatologyLife Quality lndex,CDLQI)评价两组患者生活质量,4周为1个疗程。结果:连续治疗4周,观察组患者的症状及体征评分均显著低于对照组,统计学方法,差异有统计学意义(P<0.01);观察组临床总有效率为(83.87%,26/31),明显高于对照组的(58.06%,18/31),差异具有统计学意义(P<0.01);观察组症状或感觉、工作或学习、休闲活动、睡眠及治疗影响等方面得到明显改善,优于对照组,差异有统计学意义(均P<0.01);两组人际关系改善比较,无统计学意义(P>0.05);观察组不良反应发生率明显低于对照组(P<0.01)。结论:中药止痒散外洗剂治疗特应性皮炎,具有明显的临床疗效,可以改善患者的生活质量,在临床值得推广与应用。

Objective:To investigate the clinical effect of Zhiyang powder(止痒散)external lotion in the treatment of atopic dermatitis(AD)and to evaluate the quality of life of the patients,and provide evidence for clinical treatment.Methods:62 cases of atopic dermatitis patients who had been diagnosed in Puning People's Hospital of Jieyang City,Guangdong Province were selected and divided into the observation group(31 cases)and the control group(31 cases)according to the random number table method.The control group was given clean fluticasone propionate treatment,the observation group were given Zhiyang powder external lotion treatment.After treatment,skin lesions and pruritus were observed in the two groups.The efficacy of the two groups was evaluated by SCORAD,and the quality of life of the two groups was evaluated by CDLQI(Pediatric Dermatology Quality of Life Index),with 4 weeks as a course of treatment.Results:After 4 weeks of continuous treatment,the scores of symptoms and signs in the observation group were significantly lower than those in the control group,which was with statistically significant differences(P<0.01).The total clinical effective rate in the observation group was(83.87%,26/31)which was significantly higher than that in the control group(58.06%,18/31),which was with statistically significant difference(P<0.01).In the observation group,symptoms or feelings,work or study,leisure activities,sleep and treatment effect were significantly improved,which was better than that of the control group,which was with statistically significant differences(all P<0.01).There was no statistical significance in the improvement of interpersonal relationship between the two groups(P>0.05).The incidence of adverse reactions in the observation group was significantly lower than that in the control group(P<0.01).Conclusion:Zhiyang powder external lotion has obvious clinical efficacy in the treatment of atopic dermatitis and it can improve the quality of life of the patients.It is worthy of promotion and application in clinical practice.
