耳内镜下外耳道冲洗联合派瑞松治疗真菌性外耳道炎的疗效观察

Efficacy of external ear canal irrigation combined with Pevisone in the treatment of fungal otitis externa under otoscope

目的观察真菌性外耳道炎应用耳内镜下外耳道冲洗联合曲安奈德益康唑乳膏(商品名:派瑞松)治疗的疗效。方法 100例真菌性外耳道炎患者,随机分为观察组和对照组,各50例。观察组采用耳内镜下外耳道冲洗联合派瑞松治疗,对照组采用耳内镜下外耳道冲洗联合硝酸咪康乳膏治疗。比较两组患者治疗效果、不良反应发生情况、临床症状改善时间。结果观察组治疗总有效率96.0%显著高于对照组的80.0%,差异具有统计学意义(P<0.05)。观察组患者不良反应发生率4.0%(2/50)低于对照组的18.0%(9/50),差异具有统计学意义(P<0.05)。观察组耳痛、耳鸣、局部瘙痒、耳闷改善时间分别为(4.8±2.6)、(5.6±2.2)、(6.8±2.6)、(5.9±2.7)d,均明显短于对照组的(6.5±2.3)、(7.8±2.3)、(8.6±3.1)、(8.2±3.3)d,差异具有统计学意义(P<0.05)。结论真菌性外耳道炎应用耳内镜下外耳道冲洗联合派瑞松治疗时具有显著的治疗效果,可快速缓解耳痛、耳痒等症状,且不良反应较少,安全性较高。

Objective To observe the efficacy of external ear canal irrigation combined with triamcinolone acetonide and econazole nitrate cream(trade name:Pevisone)in the treatment of fungal otitis externa under otoscope.Methods A total of 100 patients with fungal otitis externa were randomly divided into observation group and control group,with 50 cases in each group.The observation group was treated with external ear canal irrigation and Pevisone under otoscope,and the control group was treated with external ear canal irrigation and miconazole nitrate cream.The therapeutic effect,occurrence of adverse reactions,and improvement time of clinical symptoms were compared between the two groups.Results The total effective rate 96.0%of the observation group was significantly higher than 80.0%of the control group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions 4.0%(2/50)of the observation group was lower than 18.0%(9/50)of the control group,and the difference was statistically significant(P<0.05).The improvement time of earache,tinnitus,local itching,and ear fullness of the observation group were(4.8±2.6),(5.6±2.2),(6.8±2.6)and(5.9±2.7)d,which were obviously shorter than(6.5±2.3),(7.8±2.3),(8.6±3.1)and(8.6±3.1)d of the control group,and the difference was statistically significant(P<0.05).Conclusion Combination of external ear canal irrigation and Pevisone under otoscope has remarkable therapeutic effect on patients with fungal otitis externa,and it can relieve earache and itching quickly,and has less adverse reactions and higher safety.
