老年泌尿外科疾病患者并发糖尿病围术期治疗的临床研究

Clinical Study of Perioperative Treatment for Elderly Patients with Urological Diseases Complicated with Diabetes

目的探究老年泌尿外科疾病患者并发糖尿病围术期治疗的临床效果。方法选取2019年1月—2020年6月该院收治的126例老年泌尿外科疾病并发糖尿病患者,随机分成参照组和研究组,各63例。两组患者均接受泌尿外科手术治疗,参照组在此基础上行围术期常规治疗,研究组行围术期针对性治疗。对比两组治疗效果、血糖水平、并发症、治疗工作评分、生活质量评分。结果①研究组治疗总有效率高于参照组(96.83%>87.30%),差异有统计学意义(χ^(2)=3.910,P=0.048);②治疗前,两组血糖情况对比,差异无统计学意义(P>0.05);治疗后,研究组FPG、2 hPG、HbA1c水平低于参照组,差异有统计学意义(P<0.05);③研究组并发症发生率低于参照组(6.35%<19.05%),差异有统计学意义(χ^(2)=4.582,P=0.032);④研究组治疗工作评分高于参照组,差异有统计学意义(P<0.05)。⑤研究组生活质量评分比对照组高,差异有统计学意义(t=2.111,P=0.037)。结论老年泌尿外科疾病患者并发糖尿病予以围术期针对性治疗,效果确切,能够控制患者血糖水平,降低并发症发生率,且治疗工作评分较高,有助于提高临床疗效,推荐开展。

Objective To explore the clinical effects of perioperative treatment of elderly patients with urological diseases complicated by diabetes.Methods A selection of 126 elderly patients with urological diseases complicated by diabetes who were admitted to the hospital from January 2019 to June 2020.Urinary patients were randomly divided into a reference group and a study group,with 63 cases in each group.Both groups of patients received urological surgical treatment.The reference group performed perioperative routine treatment on this basis,and the study group received perioperative targeted treatment.The treatment effect,blood glucose level,complications,treatment work score and quality of life score were compared between the two groups.Results 1.The total effective rate of treatment in the study group was higher than that in the reference group(96.83%>87.30%).The difference was statistically significant(χ^(2)=3.910,P=0.048).2.Before treatment,there was no statistically significant difference in blood glucose between the two groups(P>0.05);after treatment,the study group of the levels of FPG,2 hPG and HbA1c were lower than those of the reference group.The difference was statistically significant(P<0.05).3.The incidence of complications in the study group was lower than that of the reference group(6.35%<19.05%).The difference was statistically significant(χ^(2)=4.582,P=0.032).4.The treatment scoring of the study group was higher than the reference group.The difference was statistically significant(P<0.05).5.The quality of life score of the study group was higher than that of the control group.The difference was statistically significant(t=2.111,P=0.037).Conclusion For elderly patients with urological diseases complicated with diabetes,targeted treatment during perioperative period has a definite effect.It can control the blood glucose level of patients and reduce the incidence of complications.The treatment score is high,which helps to improve the clinical efficacy and is recommended.
