从用药咨询在药学门诊的实践看药学服务的重要性

The importance of pharmaceutical care from the practice of drug consultation in pharmacy outpatient department in our hospital

目的从用药咨询在药学门诊的实践看药学服务的重要性。方法选择2017年7月~2019年6月期间门诊患者药物咨询记录498份,参考药品说明书、《中国药典》(2015版)等相关药物专业书籍,分析用药咨询患者的基本信息、咨询药物类别、咨询药物具体类型。结果门诊药物咨询以女性患者居多,共有376例,占75.5%;从年龄来看≥60岁患者最多,占50.8%;在使用药物种类上分析,同时使用2种及以下患者占45.4%,使用3~4种药物患者占41.5%,5种以上的药物的占13.1%;心血管系统和内分泌系统用药咨询居于前列,分别为236份占27.2%,224份占25.8%,呼吸系统和消化系统用药位居其次,分别为156份占18.0%,132份占15.2%;在咨询问题中,药物用量用法咨询比例为34.8%,药理作用与用途咨询比例为26.8%,用药注意事项咨询比例为10.6%,咨询方式以当面咨询为主。结论用药咨询作为药学服务的突破口,根据药学服务存在的问题和需求,不断地制订有效措施加以改进,对医疗质量的提高发挥了重要的作用,解决了临床医师的后顾之忧。

Objective The importance of pharmaceutical care is seen from the practice of medication consultation in the pharmacy clinic.Methods Select 498 medical consultation records of outpatients from July 2017 to June 2019,refer to the drug instructions,‘Chinese Pharmacopoeia’(2015 edition)and other relevant professional books on medicine,and analyze the basic information and consultation of the patients in our hospital.Drug category,specific type of consulting drug.Results The majority of outpatient drug consultations in this hospital were female patients,with a total of 376 cases,accounting for 75.5%;in terms of age,patients≥60 years old accounted for the most,accounting for 50.8%;in terms of the types of drugs used,patients using 2 or less at the same time accounted for 45.4%Patients using 3 to 4 drugs accounted for 41.5%,and more than 5 drugs accounted for 13.1%.Cardiovascular system and endocrine system medication consultations were at the forefront,with 236 cases accounting for 27.2%,224 cases accounting for 25.8%,respiratory system and digestive system medications ranked second,with 156 medications accounting for 18.0%and 132 medications accounting for 15.2%.Among the consultation questions,the proportion of consultations on drug dosage and usage was 34.8%,the proportion of consultations on pharmacological effects and uses was 26.8%,and the proportion of consultations on medication precautions The rate is 10.6%,and the consultation method is mainly face-to-face consultation.Conclusion Medication consultation is a breakthrough in pharmaceutical services.According to the existing problems and needs of pharmaceutical services,our hospital has continuously formulated effective measures to improve it.This has played an extremely important role in improving the quality of medical care and solved the worries of clinicians.
