结直肠癌腹腔镜手术42例分析

Application of Laparoscopy in Colorectal Cancer Surgery:An Analysis of 42 Cases

目的探讨结直肠癌腹腔镜手术较开放手术的优势。方法对安徽省滁州市中西医结合医院腹腔镜辅助下结直肠癌手术患者和既往传统开放手术的结直肠癌患者的临床资料进行回顾性分析,分析比较两组患者术中、术后相关情况及术后近期疗效。结果腹腔镜手术组术中出血量、手术时间及术后镇痛情况与传统手术组比较,出血少,术后使用镇痛药少,手术时间稍长(P均<0.05)。术后两组患者淋巴结清扫数目[腹腔镜组为(31.26±2.43)个,传统手术组为(28.78±2.12)个]及肠管切除长度[腹腔镜组为(16.29±1.46)cm,传统开腹组为(18.32±1.37)cm]比较差异均无统计学意义(P均>0.05)。术后肠道功能(术后排气,术后排便、术后进食时间)腹腔镜组较传统手术组恢复快(P均<0.05)。术后并发症发生率腹腔镜组较传统手术组低(P<0.05)。术后住院时间腹腔镜组较传统手术组短(P<0.05)。结论腹腔镜结直肠癌手术较传统开放手术具有出血少,术后疼痛轻,术后恢复快,术后并发症少,住院时间短的优势。

Objective To explore the advantages of laparoscopic surgery for colorectal cancer compared with open abdominal surgery.Methods The clinical data of patients with laparoscopic assisted colorectal cancer and patients with traditional open abdominal surgery in Chuzhou Hospital of Integrated Traditional Chinese and Western Medicine hospital were analyzed retrospectively.Results Compared with the traditional operation group,the laparoscopic operation group had less bleeding(P<0.05),less analgesia(P<0.05)and longer operation time(P<0.05).The number of lymph node dissection[(31.26±2.43)in laparoscopic group and(28.78±2.12)in traditional operation group]and the length of bowel resection[(16.29±1.46)cm in laparoscopic group and(18.32±1.37)cm in traditional laparotomy group]were different between the two groups after operation(P all>0.05).Postoperative intestinal function recovery(postoperative exhaust,postoperative defecation and postoperative eating time)in laparoscopic group was faster than that in traditional operation group(P all<0.05).The incidence of postoperative complications in laparoscopic group was lower than that in traditional operation group(P<0.05).The postoperative hospital stay in laparoscopic group was shorter than that in traditional operation group(P<0.05).Conclusion Compared with traditional open surgery,laparoscopic colorectal cancer surgery has the advantages of less bleeding,less postoperative pain,faster recovery,fewer postoperative complications and shorter hospital stay.
