血浆脑钠肽水平在慢性心力衰竭的诊断、病情及预后评估中的应用价值 被引量:3

摘要 目的:探究血浆脑钠肽(Brain Natriuretic Peptide,BNP)水平在慢性心力衰竭的诊断、病情及预后评估中的应用价值。方法:选取62例慢性心力衰竭患者为研究对象,62例患者采用美国纽约心脏病协会(New York Heart Association,NYHA)分级法进行心功能分级,并检测患者的血浆BNP水平、左室舒张末期内径(Left Ventricular End Diastolic Diameter,LVEDD)与左室射血分数(Left Ventricular Ejection Fraction,LVEF),比较不同心功能分级患者的血浆BNP水平以及患者治疗前后的血浆BNP水平;采用SPSS Pearson相关性分析软件对慢性心力衰竭患者血浆BNP水平与LVEDD、LVEF之间的关系进行相关性分析。结果:IV级慢性心力衰竭患者血浆BNP、LVEDD水平高于II级和III级(P<0.05),LVEF水平低于II级和III级(P<0.05);III级慢性心力衰竭患者血浆BNP、LVEDD水平高于II级(P<0.05);IV级慢性心力衰竭患者的LVEF水平低于III级(P<0.05)。治疗后患者的血浆BNP水平明显低于治疗前,差异具有统计学意义(P<0.05)。SPSS Pearson相关性分析结果表明:慢性心力衰竭患者血浆BNP水平与LVEDD水平呈正相关性(P<0.05);与LVEF水平呈负相关性(P<0.05)。结论:针对慢性心力衰竭患者,监测其血浆BNP水平能准确诊断病情,并能为患者的预后提供可靠的参考依据。 Objective:To explore the application values of plasma BNP levels in the diagnosis,condition and prognosis evaluation of CHF.Methods:A total of 62 cases were selected as the research objects,and were classified by the NYHA grading method.The plasma BNP level,LVEDD and LVEF were detected.Plasma BNP levels of patients with different cardiac function classes,and the plasma BNP levels of the patients before and after were compared.The correlation between plasma BNP levels and LVEDD,and LVEF in patients were analyzed by SPSS pearson software.Results:Plasma BNP and LVEDD levels in patients with grade IV CHF were higher than those in grade II and III CHF patients(P<0.05),and LVEF levels were lower than those in grade II and III patients(P<0.05).Plasma BNP and LVEDD levels in patients with grade III CHF were higher than those in grade II patients(P<0.05).LVEF levels of grade IV patients were lower than those in grade III patients(P<0.05).The plasma BNP level of patients after treatment was significantly lower than before treatment,and the difference was statistically significant(P<0.05).The results of SPSS Pearson correlation analysis showed that the plasma BNP levels in patients with CHF was positively correlated with the LVEDD levels(P<0.05),and was negatively correlated with the LVEF levels(P<0.05).Conclusion:For patients with CHF,monitoring their plasma BNP levels can accurately diagnose the condition,and provide a reliable reference for the prognosis of patients.
