摘要
为探讨彩色多普勒超声应用于小儿肠重复畸形诊断中的价值,回顾2015年8月至2020年1月我院收治的且经术后病理检查确诊为肠重复畸形的23例患儿资料,患儿术前均行彩色多普勒超声检查,分析超声诊断情况及不同类型肠重复畸形的超声表现。结果显示,23例肠重复畸形患儿中,术前确诊18例(78.26%),误诊为梅克尔憩室2例(8.70%)、肠套叠1例(4.35%),漏诊2例(8.70%)。18例术前超声确诊的肠重复畸形患儿中,发生于直肠及乙状结肠3例,回肠10例,空肠2例,十二指肠3例。根据超声图像表现将18例肠重复畸形分型,其中包块型4例,憩室型4例,囊肿型10例;不同类型肠重复畸形超声表现各异。结果表明,彩色多普勒超声对小儿肠重复畸形具有一定的诊断价值,且具有无痛、无创、可重复、经济等优点,可作为肠重复畸形的首选筛查方法。但此法仍有一定的漏诊、误诊率,临床应总结超声检查经验,提高诊疗技术,同时配合进一步检查,以提高术前诊断率。
This study was to investigate the value of color doppler ultrasound(CDU)for diagnosing infantile interstinal duplication(IID),reviewed the data of 23 IID pediatric patients who had been treated in author’s hospital,then confirmed by using postoperative pathological exam.,before surgery they all received CDU inspection,based on the data,analysed ultrasound inspection status,and ultrasound shows of IID at different patterns.As results,before surgery in the 23 cases,18 were confirmed(78.26%),2 were misdiagnosed as Meckl’s diverticulum(8.7%),1 as introsusception(4.35%),2 were missed diagnosis(8.70%);in above confirmed 18 cases 3 suffered from retum and sigmoid,10 from ilem,2 from jejuum,and 3 from duodenum which if according to image shows of above IID patterns,4 cases belonged to mass-type,4 to diverticulum-type,10 to cyst-type.Results show that CDU in diagnosing IID,has such advantages as painless,non-trauma,repeatable,and economic,etc,thus it could be first choice for screening IID,but,there still is certain ratio of miadiagnosed cases and missed diagnosis,therefore,clinically it should be to summarize inspection experience,to lift diagnostic&treatment skill,at the same time also to make furture inspection,so that enhance preoperative diagnostic rate.
