苯磺酸氨氯地平片联合琥珀酸美托洛尔缓释片治疗老年高血压患者的临床疗效

Clinical Effects of Amlodipine Besylate Tablets Combined with Metoprolol Succinate Sustained Release Tablets in the Treatment of Elderly Patients with Hypertension

目的探讨苯磺酸氨氯地平片联合琥珀酸美托洛尔缓释片治疗老年高血压患者的临床疗效。方法选取2019年12月至2020年8月在佛山市禅城区人民医院接受治疗的老年高血压患者90例作为研究对象,采用双色球法随机分为对照组与观察组,每组45例。入组后两组患者先停止使用原降压药物,以安慰剂洗脱治疗2周后,对照组给予琥珀酸美托洛尔缓释片,观察组在此基础上加用苯磺酸氨氯地平片。比较两组患者临床疗效、血压变化情况、心功能指标、肾功能指标及不良反应发生情况。结果观察组治疗有效率为95.56%,高于对照组的82.22%,差异有统计学意义(P<0.05)。治疗后两组患者24 h平均收缩压、24 h平均舒张压较治疗前下降,且观察组下降幅度更大,差异有统计学意义(P<0.05)。治疗后两组患者左室射血分数较治疗前升高,观察组升高幅度更大,治疗后两组左室舒张末期内径及左室收缩末期内径较治疗前下降,观察组下降更明显,差异有统计学意义(P<0.05)。治疗后两组患者24 h尿微量白蛋白及尿白蛋白与肌酐比值较治疗前下降,且观察组下降幅度更大,差异有统计学意义(P<0.05)。两组不良反应发生率比较差异无统计学意义(P>0.05)。结论苯磺酸氨氯地平片联合琥珀酸美托洛尔缓释片较单独使用苯磺酸氨氯地平片可明显提高老年高血压患者临床疗效,在降压及改善心功能、肾功能方面有明显的优势。

Objective To explore the clinical effects of amlodipine besylate tablets combined with metoprolol succinate sustained-release tablets in elderly patients with hypertension.Methods A total of 90 elderly hypertensive patients admitted to the People’s Hospital of Chancheng from December 2019 to August 2020 were selected as the research objects and were randomly divide into a control group and an observation group by double color ball method,with 45 cases in each group.After enrollment,the 2 groups of patients first stopped using the original antihypertensive drugs,and after two weeks of placebo-eluted treatment,the control group was given metoprolol succinate sustained-release tablets,and the observation group was added with ammonia chloride besylate on this basis of Amlodipine Besylate Tablets.The clinical efficacy,blood pressure changes,cardiac function indexes,renal function indexes and adverse reactions were compared between the 2 groups of patients.Results The total effective rate of the observation group was 95.56%,which was higher than 82.22%of the control group,the difference was statistically significant(P<0.05).After treatment,the 24-hour mean systolic blood pressure and 24-hour mean diastolic blood pressure of the 2 groups decreased compared with those before treatment,but the decrease of the observation group was greater,and the difference was statistically significant(P<0.05).After treatment,the left ventricular ejection fraction of the 2 groups of patients increased compared with that before treatment,and the increase was greater in the observation group.After treatment,the left ventricular end diastolic diameter and left ventricular end systolic diameter decreased compared with before treatment in the 2 groups.The difference was statistically significant(P<0.05).After treatment,the 24-hour urine microalbumin and the ratio of urine albumin to creatinine of the 2 groups of patients decreased compared with before treatment,and the decrease was greater in the observation group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the 2 groups(P>0.05).Conclusion Amlodipine Besylate tablets combined with metoprolol succinate sustained-release tablets can significantly improve the clinical efficacy of amlodipine besylate tablets in elderly patients with hypertension,and has obvious advantages in reducing blood pressure and improving heart function and kidney function.
