摘要 目的应用品管圈活动来降低可穿戴医疗设备故障率。方法选择南京市第一医院内分泌科320台可穿戴医疗设备为研究对象,调查2018年10月至2019年10月可穿戴医疗设备发生故障的数据292次。于2019年10月至2020年1月、2020年5月至7月实行品管圈管理,收集可穿戴医疗设备发生故障的数据。组建品管圈小组,通过头脑风暴法、甘特图、柏拉图、鱼骨图、查检表、要因分析法、雷达图等质量管理工具选定主题、把握现状、现场查检、要因分析、真因验证、实施对策、标准化巩固成果。结果在所有的故障类型中数据缺失高达240次,占故障总次数的82.19%。按照"二八法则"确定数据缺失为该次品管圈的改善重点。通过综合分析圈员的工作年限、学历改善能力、主题改善能力确定圈能力为79.01%。经过计算确定改善的目标值为0.088%,改善幅度为-64.80%,需要在实施品管圈前可穿戴医疗设备故障率的基础上降低64.80%。实施对策后,可穿戴医疗设备故障率由改善前的0.250%降低到改善后的0.058%,目标达标率为118.51%,进步率76.80%,1个月效果维持期故障率为0.060%,对品管圈的成果进行标准化。结论品管圈活动充分发挥医学工程部门和临床科室结合的优势,达到了预期效果,明显降低了可穿戴医疗设备故障率,对类似医疗设备的质量安全管理有一定借鉴意义。在活动中圈员们积极发散思维、互帮互助,提高了个人能力和团队协作精神,增强了"以病人为中心、以临床为重点"的服务理念,积极为医院的安全有效运行保驾护航。 Objective To reduce failure rate of wearable medical equipment by quality control circle activities.Methods A total of 320 wearable medical devices of Endocrinology Department in Nanjing First Hospital were selected and investigated,the failure numbers wearable medical devices were 292 from October 2018 to October 2019.From October 2019 to January 2020,and from May to July in 2020,quality control circle management were conducted and then the failure numbers of wearable medical equipment was calculated,respectively.The quality control circle team was formed,and quality management tools including brainstorming,Gantt chart,Plato,fish-bone diagram,checklist,factor analysis method and radar chart were used to carry out topic selection,status quo analysis,site inspection,cause analysis,verification,implementation countermeasures and standardization consolidation.Results Data missing was up to 240 times in all failure types,which accounted for 82.19%of total failures.Data missing was the improvement focus of quality control circle in accordance with“Twenty-eight Rule”.The work seniority,academic improvement ability and theme improvement ability of circle members were analyzed to determine that circle ability was 79.01%.The target value of improvement was 0.088%after calculation,the improvement rate was-64.80%,which means the failure rate of wearable medical equipment was aimed to reduced by 64.80%on the basis of quality control circle.After quality control strategy was conducted,failure rate of wearable medical equipment was reduced from 0.250%to 0.058%,target compliance rate was 118.51%,progress rate was 76.80%,failure rate at one-month maintenance period was 0.060%and quality control circle results were standardized.Conclusion It is demonstrated that quality control circle activities give full play to the advantages of medical engineering department and clinical departments,which can significantly reduce failure rate of wearable medical equipment,and can be used as the reference for quality and safety management of similar medical equipment.Through the activity,circle members are actively participated and collaborated,which improve personal and teamwork ability,enhance service concept of“patient-centered,clinical-focused”,and ensure the safe and effective operation of hospital.
