摘要
目的探讨宫颈癌组织高危型人乳头瘤病毒(hrHPV)感染与凋亡相关因子(Fas)、含半胱氨酸天冬氨酸蛋白水解酶3(Caspase-3)及Caspase-8蛋白表达的关系。方法选取重庆市大足区人民医院2016年6月-2019年5月收治的宫颈癌行手术治疗患者91例,收集对应的术后病理组织,采用PCR反向杂交法检测宫颈癌组织和癌旁正常组织中hrHPV的感染情况,采用免疫组化法检测Fas、Caspase-3与Caspase-8蛋白表达,分析宫颈癌组织中hrHPV感染与Fas、Caspase-3与Caspase-8蛋白表达的关系。结果 91例宫颈癌组织中有72例发现hrHPV感染,感染阳性率为79.12%;91例癌旁正常组织中仅有3例发现hrHPV感染,感染阳性率为3.30%;宫颈癌组织中hrHPV阳性率高于癌旁正常组织,差异有统计学意义(χ^(2)=107.975,P<0.05)。宫颈癌组织中Fas、Caspase-3与Caspase-8阳性率低于癌旁正常组织,差异均有统计学意义(χ^(2)=30.102、61.088、64.096,P<0.05)。hrHPV感染宫颈癌组织中Fas、Caspase-3与Caspase-8蛋白表达量明显低于无hrHPV感染宫颈癌组织,差异有统计学意义(χ^(2)=26.167、34.232、32.856,P<0.05)。单因素方差分析显示,宫颈癌hrHPV感染与年龄、临床分期、病理分型、组织学分级和淋巴结转移均无关(χ^(2)=2.367、1.281、0.272、1.027、0.274,P均>0.05),Fas、Caspase-3及Caspase-8蛋白表达与临床分期、组织学分级有关(Fas:χ^(2)=5.929、4.540;Caspase-3:χ^(2)=5.230、5.429;Caspase-8:χ^(2)=4.355、4.326;P均<0.05)。Spearman秩相关检验发现,hrHPV感染与Fas、Caspase-3、Caspase-8蛋白表达量均呈显著负相关(r=-0.503、-0.582、-0.514,P<0.05)。结论 hrHPV感染与早期宫颈癌变密切相关,其可能是通过抑制Fas、Caspase-3与Caspase-8蛋白的表达来促使宫颈细胞癌变。
Objective To investigate the expression of factor associated suicide(Fas),cystein aspartic acid specific protease-3(Caspase-3)and Caspase-8 in cervical cancer with high-risk human papillomavirus(hrHPV)infection. Methods The clinical data of 91 patients with cervical cancer who were operated in the People’s Hospital of Dazu from June 2016 to May2019 were analyzed retrospectively. The corresponding postoperative pathological tissues were collected. The infection of hrHPV in cervical cancer and normal cervical tissues adjacent to cancer was detected by PCR reverse hybridization. The expression of Fas,Caspase-3 and Caspase-8 protein were detected by immunohistochemistry,and the sensitivity of hrHPV in cervical tissues was analyzed. The relationship between Fas,Caspase-3 and Caspase-8 protein expression was analyzed.Results hrHPV infection was found in 72 of 91 cases of cervical cancer,and the positive rate was 79.12%;only 3 of 91 cases of adjacent tissues were positive,with the positive rate of 3.30%. The positive rate of hrHPV in cervical cancer was higher than that in normal tissues(χ^(2)=107.975,P<0.05). The positive rates of Fas,Caspase-3 and Caspase-8 in cervical cancer were lower than those in normal tissues(χ^(2)=30.102,61.088,64.096,P<0.05). The expression of Fas,Caspase-3,Caspase-8 and Bcl-2 in cervical cancer with hrHPV infection was significantly lower than those in non hrHPV infection(χ^(2)=26.167,34.232,32.856,P<0.05). Single factor analysis of variance showed that hrHPV infection was not related to age,clinical stage,pathological classification,degree of tissue differentiation and lymph node metastasis(χ^(2)=2.367,1.281,0.272,1.027,0.274,P>0.05). Fas,Caspase-3 and Caspase-8 protein expression were related to clinical stage and degree of tissue differentiation(Fas:χ^(2)=5.929,4.540;Caspase-3:χ^(2)=5.230,5.429;Caspase-8:χ^(2)=4.355,4.326;P<0.05). Spearman’s rank correlation test showed that the expression of Fas,Caspase-3 and Caspase-8 protein was negatively correlated with hrHPV infection(r=-0.503,-0.582,-0.514,P<0.05). Conclusions hrHPV infection was closely related to early cervical carcinogenesis.It might promote cervical carcinogenesis by inhibiting the expression of Fas,Caspase-3 and Caspase-8 protein.
