药罐联合中药热敷治疗气滞血瘀型腰椎间盘突出症的临床研究

Study on the clinical effects of drug cupping combined with tradional Chinese medicine hot compress on lumbar disc herniation with Qi stagnation and blood stasis

目的:评价药罐联合中药热敷治疗气滞血瘀型腰椎间盘突出症的有效性。方法:纳入102例气滞血瘀型腰椎间盘突出症保守治疗病人,按随机数表法分为对照组36例,试验A组32例,试验B组34例。对照组给予静脉输液以及常规理疗(中频理疗、耳穴埋豆等),试验A组在对照组基础上给予常规中医玻璃拔罐法治疗及中药热敷,试验B组在对照组基础上给予中药药罐拔罐法治疗及中药热敷。3组病人均治疗2周。比较3组病人各治疗阶段的视觉模拟评分法(VAS)评分、Oswestry功能障碍指数问卷表(ODI)评分、中医证候评分及临床疗效。结果:3组治疗前VAS评分及ODI评分比较差异无统计学意义(P>0.05);试验B组治疗后3 d、1周及2周的VAS评分明显低于对照组(P<0.05);试验B组的治疗总有效率为97.10%,高于对照组和试验A组(P<0.05)。结论:药罐联合中药热敷治疗腰椎间盘突出症(气滞血瘀型)有效,可减轻病人疼痛程度,缓解中医症状,改善肢体活动。

Objective:To evaluate the effectiveness of drug cupping combined with tradional Chinese medicine heat compress in the treatment of lumbar disc herniation with Qi stagnation and blood stasis type.Methods:One hundred and two lumbar disc herniationwith Qi stagnation and blood stasis type patients treated with conservative method were divided into the control group(36 cases),group test A(32 cases)and group test B(34 cases)according to the random number table method.The control group was treated with intravenous infusion and conventional physiotherapy(medium frequency physiotherapy,ear acupoint embedding,etc.),the group test A were treated with traditional Chinese medicine cupping and traditional Chinese medicine hot compress on the basis of the control group,and the group test B were treated with traditional Chinese medicine cupping and tradional Chinese medicine hot compress medicine on the basis of the control group.Three groups were treated for 2 weeks.The visual analogue scale(VAS)score,Oswestry disability index(ODI)score,tradional Chinese medicine(TCM)syndrome score and clinical efficacy among three groups were compared.Results:The differences of the VAS score and ODI score among three groups before treatment were not statistically significant(P>0.05).After 3 days,1 week and 2 weeks of treatment,the VAS score and ODI score in group test B were significantly lower than that in control group and group test A(P<0.05).After 2 weeks of treatment,the TCM syndrome score in group test B were significantly lower than that in control group and group test A(P<0.01).The total effective rate in group test B(97.10%)was significantly higher than that in control group and group test A(P<0.05).Conclusions:The medicine cupping combined with tradional Chinese medicine heat compress in the treatment of lumbar disc herniation with Qi stagnation and blood stasis type is effective,and which can alleviate pain,relieve TCM symptoms,and improve limb movements.
