脑干胶质瘤MRI征象与临床病理分级的相关性研究

Correlation between MRI Signs of Glioma of the Brain Stem and Clinical-pathological Grade

目的旨在探讨脑干胶质瘤MRI征象与临床病理分级的相关性。方法选取我院2017年6月至2019年1月收治的脑胶质瘤患者67例(低级别者42例、高级别者25例),收集53例患者MRI扫描图像及临床资料,对比不同病理分级脑干胶质瘤患者MRI图像表现。结果两组患者年龄、性别、文化程度、发病部位资料比较无明显差异(P>0.05),不同级别脑干脑胶质瘤患者表现存在明显差异(P<0.05),高级别组患者多见基底动脉包绕、坏死、肿瘤直径多≥2cm、呈现跨中线生长方式,增强扫描中强化形态主要以花环样、壁结节不均匀为主,低级别组强化程度显著低于高级别组(P<0.05);低级别组MRI测量最大厚度、最大厚度比值/差值及最小厚度比值/差值水平明显低于高级别组,差异有统计学意义(P<0.001);两组患者最小厚度值比较无明显差异(P>0.05)。结论MRI可有效清晰显示脑干胶质瘤影像学表现,根据MRI征象可评估患者临床病理分级,为临床提供可靠影像学信息。

Objective To explore the correlation between MRI signs of glioma of the brain stem and clinicalpathological grade.Methods 67 patients with glioma of brain stem admitted to our hospital from June2017 to January 2019(42 patients in low grade and 25 patients in high grade) were selected.MRI scan images and clinical data of 53 patients were collected to compare the MRI images of patients with different pathological grades of brainstem glioma.Results There were no significant differences in age,gender,education,and attack location between the two groups(P>0.05).There were significant differences in the performance of patients with different pathological grades of brainstem glioma(P<0.05).Patients in the high-grade group were more likely to be seen with encompassment and necrosis of basilar artery,tumor diameter ≥ 2 cm, and growth pattern across the midcourt line.In the enhanced sca n,the reinforcement shape was mainly ga rland-like,and the mural nodule was uneven.The degree of enhancement in the low-level group was significantly lower than that in the high-level group(P<0.05).The maximum thickness,maximum thickness ratio/difference,and minimum thickness ratio/difference level measu red by MRI in the low-grade group we re significantly lower than those in the high-grade group,and the diffe rence was statistically significant(P<0.001).The re was no significa nt difference in the minimum thickness between the two groups(P>0.05).Conclusion MRI can effectively display the imaging findings of brainstem glioma,and the clinical-pathological grade can be evaluated according to MRI signs,which ca n provide reliable imaging information for the clinic.
