摘要
目的:调查与分析专科护士临床实践的现状。方法:采用自行设计的专科护士回归临床实践调查表对已接受专科护士培训的人员78名进行问卷调查,系统调查专科护士临床实践现状以及综合能力提升情况。其次,通过系统比较任务清单式管理模式实施前后专科护士回归临床最佳实践执行率的差异,以期系统评估任务清单式管理模式的临床应用潜力。结果:专科护士临床实践存在一定不足,例如认为专科护士角色定位不清晰11.3%,认为专职工作岗位不明确5.6%,认为岗位津贴不达标等15.5%,认为临床科研开展不到位7.0%,认为临床重视程度不够7.0%。入组专科护士的综合能力提升情况各不相同,其中认为临床实践得到提升达53.6%,但认为对专科发展无积极作用为45.1%。任务清单式管理模式实施后的专科护士回归临床最佳实践执行率为71.8%,显著高于任务清单式管理模式实施前的30.2%,差异有统计学意义(P<0.05)。结论:专业化任职评价标准体系客观,全面揭示了当前专科护士临床实践现状的短板以及专科护士回归临床最佳实践执行率低的原因,而任务清单式管理模式能充分发挥最佳实践,具有较大的临床应用价值。
Objective:To investigate and analyze the current status of the clinical practice of specialized nurses.Methods:A self-designed questionnaire was used to investigate 78 nurses who had received the training of specialist nurses.Systematically investigated the clinical practice status and comprehensive ability improvement of specialized nurses.Secondly,by systematically comparing the differences in the execution rate of specialized nurses returning to clinical best practice before and after the implementation of the task-list management mode,in order to systematically evaluate the clinical application potential of the task-list management mode.Results:There were certain deficiencies in the clinical practice of specialist nurses,11.3% thought that the role of specialist nurses was not clear,5.6%thought that the full-time job was not clear,15.5% thought that the post allowance was not up to standard,7.0% thought that the clinical research could not be carried out,and 7.0% thought that the clinical attention was not enough.Secondly,the improvement of comprehensive ability of 78 specialist nurses was different.Among them,53.6% thought that clinical practice had been improved,but 45.1% thought that it had no positive effect on the development of specialty.The implementation rate of clinical best practice after task list management mode was 71.8%,it was significantly higher than before the implementation of the task list management mode(30.2%),the difference was statistically significant(P<0.05).Conclusion:The standard system of professional post evaluation objectively and comprehensively reveals the shortcomings of the current situation of clinical practice of specialist nurses and the reasons for the low implementation rate of specialist nurses returning to clinical best practice.The task list management mode can give full play to the best practice and has great clinical application value.
