自身免疫性胰腺炎与胰腺癌的CT鉴别诊断价值及准确性分析

The value and accuracy of CT in the differential diagnosis of autoimmune pancreatitis and pancreatic cancer

目的:探究CT扫描对自身免疫性胰腺炎(AIP)与胰腺癌(PC)的鉴别诊断价值。方法:2017年3月-2020年3月收治AIP患者50例与PC患者50例,所有患者均经过病理学确诊,并给予CT扫描检查。比较两组扫描结果、影像学特征及病灶CT测量值。结果:AIP患者与PC患者在病变类型、胰腺形态异常、胰胆管改变及胰周改变等扫描结果比较,差异有统计学意义(P<0.05)。AIP患者与PC患者在形态、密度均匀程度及扫描密度等方面也有明显的区别,差异有统计学意义(P<0.05)。AIP患者胰腺病变在平扫、动脉期、静脉期及延迟期的CT值均明显高于PC患者,差异有统计学意义(P<0.05)。结论:CT扫描在AIP与PC疾病的鉴别与诊断中具有显著的应用价值,可将其应用到二者的早期检测中,为临床提供更为可靠的参考依据。

Objective:To explore the value of CT scanning in the differential diagnosis of autoimmune pancreatitis(AIP)and pancreatic cancer(PC).Methods:From March 2017 to March 2020,50 patients with AIP and 50 patients with PC were selected.All patients were confirmed by pathology and given CT scanning.We compared the scanning results,imaging features and CT measurement of lesions between the two groups.Results:There were significant differences between AIP patients and PC patients in the types of lesions,abnormal morphology of pancreas,changes of pancreatic duct and peripancreatic changes(P<0.05).There were also significant differences in morphology,density uniformity and scanning density between AIP patients and PC patients,the difference was statistically significant(P<0.05).The CT values of pancreatic lesions in AIP patients were significantly higher than those in PC patients in plain scan,arterial phase,venous phase and delayed phase,the difference was statistically significant(P<0.05).Conclusion:CT scan has significant application value in the differential diagnosis of AIP and PC,it can be applied to the early detection of both,and provide more reliable reference for clinical.
