儿童以腹痛为首发表现的内科急危重症临床诊疗探讨

Discussion on clinical diagnosis and treatment of acute and critical diseases in children with abdominal pain as the first manifestation

目的:探讨小儿内科急危重症患儿以腹痛为首发表现时的临床特点及治疗情况,旨在提高急危重症患儿的救治成功率,改善患儿预后。方法:2015年6月-2020年6月收治急危重症患儿48例,所有患儿均以腹痛为首发表现。分析患儿的临床特点及治疗情况,总结治疗方法及经验。结果:48例患儿中,腹型过敏性紫癜19例,糖尿病酮症酸中毒11例,爆发性心肌炎12例,梗阻性休克6例;四种急症的治疗总有效率分别为94.74%、90.91%、91.67%、100.00%;死亡率分别为0、9.09%、8.33%、0。结论:对于以腹痛首发表现的儿内科急危重症患儿,应根据患儿基本情况、身体状况及诊断结果给予个体化治疗措施,才能及时挽救患儿生命,提高整体治疗效果,降低病死率。

Objective:To discuss the current clinical characteristics and treatment of children with abdominal pain as the first manifestation,so as to improve the first aid success rate of acute and critical children and improve the prognosis of children.Methods:From June 2015 to June 2020,48 cases of children with acute and critical illness were enrolled,all children had abdominal pain as the first manifestation.Analyzed the clinical characteristics and treatment situation of the children,and summarized the treatment methods and experience.Results:Among the 48 children,there were 19 cases of abdominal allergic purpura,11 cases of diabetic ketoacidosis,12 cases of fulminant myocarditis,and 6 cases of obstructive shock.The total effective rates of treatment of the four types of emergencies were 94.74%,90.91%,91.67%,and 100.00% respectively.The mortality rates were 0%,9.09%,8.33%,and 0.Conclusion:For children with acute and critical illness in pediatrics with the first manifestation of abdominal pain,individualized treatment measures should be given according to the basic condition,physical condition and diagnosis results of the children in order to save the lives of the children in time,improve the overall treatment effect,and reduce the mortality rate.
