双能量CT与多层螺旋CT评估胃癌分化程度的比较

Comparison of dual energy CT and MSCT in evaluating differentiation degree of gastric cancer

目的:对比分析胃癌患者采取双能量CT与多层螺旋CT诊断以评估分化程度的临床价值。方法:选取2019年3月—2020年12月高淳人民医院收治的胃癌确诊病患49例进行观察研究,通过诊断方式差异进行分组,双能量CT组患者接受双能量CT检查,多层螺旋CT组患者接受多层螺旋CT检查,均以手术病理结果作为金标准,探讨各组患者分化程度与结果相关性,对比分析双能量CT与多层螺旋CT评估胃癌分化程度的差异。结果:双能量CT组患者经双能量CT诊断发现,中高分化程度癌变患者NICa、NICV结果显著低于低分化程度癌变患者(P<0.05),多层螺旋CT组患者经多层螺旋CT诊断发现,中高分化程度癌变患者静脉期CT结果显著低于低分化程度癌变患者(P<0.05),但平扫期CT结果、动脉期CT结果则无统计学差异(P>0.05)。双能量CT针对不同分化程度癌变患者诊断符合率显著高于多层螺旋CT(P<0.05)。结论:利用双能量CT诊断评估胃癌患者分化程度具有准确性高、操作简单等优势,能够为临床提供可靠依据,值得推广。

Objective To compare and analyze the clinical value of dual energy CT and multislice spiral CT in the diagnosis of gastric cancer patients to evaluate the degree of differentiation.Methods Choice in March,2019 to December 2020,our hospital diagnosis of gastric cancer patients in 49 cases study,through the diagnosis way difference,divided into group dual energy CT patients receiving dual energy CT examination,group multislice CT patients underwent multislice CT examination,all results by surgery pathology as the gold standard,discussion groups of patients with differentiated results correlation,contrast analysis of dual energy computed tomography(CT)and multislice computed tomography(CT)to evaluate the degree of differentiation of gastric cancer.Results Patients in group dual energy CT of the dual energy CT found that NICa,NICV results,high differentiation degree of cancer patients significantly lower than the low degree of differentiation of cancer patients(P﹤0.05),group multislice CT patients by multislice CT found that high degree of differentiation of cancer patients with venous phase CT results significantly lower than the low degree of differentiation of cancer patients(P﹤0.05),but the scan,arterial phase CT results CT results,no statistical difference(P﹥0.05).The diagnostic coincidence rate of dual-energy CT was significantly higher than that of multi-slice spiral CT(P﹤0.05).Conclusion Dualenergy CT has the advantages of high accuracy and simple operation in the diagnosis and evaluation of the differentiation degree of gastric cancer patients,which can provide a reliable basis for clinical practice and is worth popularizing.
