冠心病慢性心力衰竭患者并发室性心律失常的临床诊疗分析

Clinical diagnosis and treatment of ventricular arrhythmia in patients with coronary heart disease and chronic heart failure

目的研究冠心病慢性心力衰竭患者并发室性心律失常的临床诊疗情况。方法72例冠心病慢性心力衰竭并发室性心律失常患者,随机分为实验组和对照组,每组36例。实验组患者行胺碘酮联合厄贝沙坦治疗,对照组患者行口服胺碘酮药物治疗。对比两组患者的临床疗效、收缩压、舒张压、心率及不良反应发生率。结果实验组治疗总有效率97.22%高于对照组的75.00%,差异有统计学意义(P<0.05)。治疗后,实验组收缩压、舒张压、心率均低于对照组,差异有统计学意义(P<0.05)。实验组不良反应发生率2.78%(1/36)低于对照组的22.22%(8/36),差异有统计学意义(P<0.05)。结论冠心病慢性心力衰竭患者并发室性心律失常患者应用胺碘酮联合厄贝沙坦效果更佳,能够迅速起效,缓解患者心律失常的临床症状,帮助患者降低血压和改善心室负荷,值得应用和推广。

Objective To study the clinical diagnosis and treatment of ventricular arrhythmia in patients with coronary heart disease and chronic heart failure.Methods A total of 72 patients with coronary heart disease and chronic heart failure complicated with ventricular arrhythmia were randomly divided into experimental group and control group,with 36 cases in each group.The experimental group was treated with amiodarone and irbesartan,and the control group was treated with miodarone.The clinical efficacy,systolic blood pressure,diastolic blood pressure,heart rate and occurrence of adverse reactions were compared between the two groups.Results The total effective rate 97.22%of the experimental group was higher than 75.00%of the control group,and the difference was statistically significant(P<0.05).After treatment,the systolic blood pressure,diastolic blood pressure and heart rate of the experimental group were lower than those of the control group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions 2.78%(1/36)of the experimental group was lower than 22.22%(8/36)of the control group,and the difference was statistically significant(P<0.05).Conclusion Combination of amiodarone and irbesartan shows remarkable effect on patients with coronary heart disease and chronic heart failure complicated with ventricular arrhythmia,which can quickly take effect,relieve the clinical symptoms of arrhythmia,help patients reduce blood pressure and improve ventricular load,which is worthy of application and promotion.
