低频rTMS刺激联合早期康复运动对缺血性脑卒中患者运动、语言功能恢复的影响 被引量:4

摘要 目的探讨低频重复经颅磁刺激(rTMS)联合早期康复运动对缺血性脑卒中患者的运动、语言功能恢复及生活质量的影响。方法选取海丰县彭湃纪念医院神经内科2019年10月至2020年10月收治的60例缺血性脑卒中患者为研究对象,采用随机数表法分为观察组和对照组,每组30例。对照组患者实施早期康复运动干预,观察组患者实施低频rTMS刺激联合早期康复运动干预,两组患者均干预2周。采用Fugl-Meyer运动量表、汉语失语成套测验(ABC)、生活质量量表(SF-36)评价患者干预前及干预2周后的运动功能、语言功能和生活质量。结果干预前,两组患者的肢体功能、语言功能、生活质量评分比较差异均无统计学意义(P>0.05);干预2周后,观察组患者的上肢运动功能、下肢运动功能评分分别为(51.28±3.14)分、(27.61±1.12)分,明显高于对照组的(46.93±3.15)分、(23.05±1.13)分,差异均有统计学意义(P<0.05);干预2周后,观察组患者的谈话、理解、复述评分分别为(70.06±3.14)分、(72.38±3.54)分、(84.17±4.06)分,明显高于对照组的(66.81±3.11)分、(67.96±3.52)分、(79.62±4.05)分,差异均有统计学意义(P<0.05);干预2周后,观察组患者的健康程度、心理卫生、躯体功能评分分别为(73.48±6.23)分、(78.31±6.37)分、(80.94±5.10)分,明显高于对照组的(65.79±6.22)分、(70.92±6.34)分、(74.55±5.09)分,差异均有统计学意义(P<0.05)。结论低频rTMS刺激联合早期康复运动能有效改善缺血性脑卒中患者的运动、语言功能,提升患者的生活质量,值得临床推广使用。 Objective To explore the effect of low-frequency rTMS(repetitive transcranial magnetic stimula-tion)combined with early rehabilitation exercise on the motor,language function recovery and quality of life in patients with ischemic stroke.Methods A total of 60 patients with ischemic stroke,who admitted to Department of Neurology,Pengpai Memorial Hospital of Haifeng County from October 2019 to October 2020,were selected and divided into the observation group and control group according to random number table method,with 30 patients in each group.The control group received early rehabilitation exercise intervention,and the observation group received low-frequency rT-MS stimulation combined with early rehabilitation exercise intervention.The patients in both groups were intervened for 2 weeks.Fugl-Meyer Exercise Scale,Chinese Aphasia Test(ABC),and Quality of Life Scale(SF-36)were used to evaluate the motor function,language function and quality of life of the patients before and 2 weeks after the intervention.Results Before the intervention,there was no significant difference in the scores of limb function,language function and quality of life between the two groups(all P>0.05);after 2 weeks of intervention,the upper limb motor function and lower limb motor function scores of the observation group were(51.28±3.14)points,(27.61±1.12)points,respectively,which were significantly higher than corresponding(46.93±3.15)points and(23.05±1.13)points of the control group(P<0.05);after 2 weeks of intervention,the scores of conversation,understanding and retelling of the observation group were(70.06±3.14)points,(72.38±3.54)points,(84.17±4.06)points,respectively,which were sig-nificantly higher than corresponding(66.81±3.11)points,(67.96±3.52)points,(79.62±4.05)points of the control group(all P<0.05);after 2 weeks of intervention,patients,health,mental health,and physical function scores were(73.48±6.23)points,(78.31±6.37)points,(80.94±5.10)points in the observation group,respectively,which were signif-icantly higher than corresponding(65.79±6.22)points,(70.92±6.34)points,(74.55±5.09)points of the control group(all P<0.05).Conclusion Low-frequency rTMS stimulation combined with early rehabilitation exercise can effectively improve the motor and language functions of patients with ischemic stroke,and improve the quality of life of patients,which is worthy of clinical application.
