袋鼠式护理联合早期抚触对早产儿营养状况和环境适应的影响

Effect of kangaroo care combined with early touch on nutritional status and environmental adaptation of premature infants

目的探讨袋鼠式护理联合早期抚触对早产儿营养状况和环境适应的影响。方法纳入2018年1月至2019年4月转运至常州市儿童医院新生儿科的早产儿86例,根据随机数字表法分为两组,各43例。对照组早产儿采用常规护理,观察组早产儿在常规护理基础上采用袋鼠式护理联合早期抚触。记录并比较两组早产儿身高、体质量、日摄入奶量和血清铁、锌、前白蛋白,以及发育商(DQ)评分。结果出生1周、出生1个月,观察组身高和体质量均显著高于对照组(P值均<0.05)。出生3 d、出生1周和出生1个月,观察组日摄入奶量均显著多于对照组(P值均<0.05)。出生1个月、3个月和9个月,观察组血清锌、铁、前白蛋白均显著高于对照组(P值均<0.05)。观察组社交行为、语言、适应性、大运动和精细动作评分均高于对照组,差异均有统计学意义(P值均<0.05)。结论袋鼠式护理联合早期抚触可改善早产儿的营养状况,提高发育和环境适应性。

Objective To explore the effect of kangaroo care combined with early touch on the nutritional status and environmental adaptation of premature infants.Methods 86 premature infants transferred to the Department of Neonatology at Changzhou Children s Hospital from January 2018 to April 2019 were enrolled.They were divided into two groups according to the random number table method,with 43 cases in each group.The control group was given routine nursing,and the observation group was given kangaroo care combined with early touch on the basis of routine nursing.The height,weight,daily intake of milk,serum iron,zinc,prealbumin and developmental quotient(DQ)score of the two groups were recorded and compared.Results At 1 week and 1 month after birth,the height and weight of the observation group were significantly higher than those of the control group(all P<0.05).At 3 days,1 week and 1 month after birth,the daily intake of milk was significantly larger than that of the control group(all P<0.05).At 1 month,3 months and 9 months after birth,the levels of serum zinc,iron and prealbumin of the observation group were significantly higher than those of the control group(all P<0.05).The scores of social behavior,language,adaptability,gross motor and fine motor of the observation group were higher than those of the control group,and the differences were statistically significant(all P<0.05).Conclusion Kangaroo care combined with early touch can improve the nutritional status of premature infants,and improve their development and environmental adaptation.
