甲强龙联合孟鲁司特钠片治疗嗜酸粒细胞增高慢阻肺急性期患者的效果探讨

Study on the Effect of Methylprednisolone Combined with Montelukast Sodium Tablets in the Treatment of Patients with Chronic Obstructive Pulmonary Eosinophilia

目的探讨甲强龙联合孟鲁司特钠在嗜酸粒细胞增高慢阻肺急性期患者治疗中的应用效果。方法该次研究78例嗜酸粒细胞增高慢阻肺急性期患者均方便选取于该院2018年3月—2019年8月收治的,按照入院时间先后分为对照组和研究组,各39例。对照组采取甲强龙治疗方案,研究组接受甲强龙联合孟鲁司特钠片治疗方案,统计各组治疗效果,并对比肺功能指标、炎症因子水平和嗜酸性粒细胞绝对值。结果研究组治疗总有效率(94.87%)较对照组(76.92%)更高,差异有统计学意义(χ^(2)=5.186,P=0.023);治疗前,两组第1秒用力呼气容量(FEV1)、用力肺活量(FVC)、降钙素原(PCT)及C反应蛋白(CRP)比较,差异无统计学意义(t=0.392、0.359、0.313、0.009,P=0.696、0.720、0.755、0.993);治疗后,研究组同对照组相比更优,差异有统计学意义(t=11.907、15.344、3.747、6.922,P<0.05)。治疗前,两组嗜酸性粒细胞绝对值比较,差异无统计学意义(t=0.023,P=0.818);治疗后,研究组比对照组低,差异有统计学意义(t=5.809,P<0.05)。结论嗜酸粒细胞增高慢阻肺患者治疗过程中,采取甲强龙联合孟鲁司特钠片治疗方案,不仅可以改善患者肺功能及炎症状况,同时可促进治疗效果提高。

Objective To explore the effect of methylprednisolone combined with montelukast sodium in the treatment of patients with chronic obstructive pulmonary disease with elevated eosinophils.Methods In this study,78 patients with eosinophilia in the acute phase of COPD admitted to the hospital were conveniently selected from March 2018 to August 2019.They were divided into a control group and a study group according to the time of admission,with 39 cases in each group.The control group took methylprednisolone treatment plan,the study group received methylprednisolone combined with montelukast sodium tablets treatment plan,the treatment effects of each group were counted,and lung function indexes,inflammatory factor levels and absolute values of eosinophils were compared.Results The total effective rate of treatment in the study group(94.87%)was higher than that in the control group(76.92%),and the difference was statistically significant(χ^(2)=5.186,P=0.023);before treatment,the forced expiratory volume in the first second(FEV1),forced vital capacity(FVC),procalcitonin(PCT)and C-reactive protein(CRP)of the two groups,the difference was not statistically significant(t=0.392,0.359,0.313,0.009,P=0.696,0.720,0.755,0.993).After treatment,the study group was better than the control group,and the difference was statistically significant(t=11.907,15.344,3.747,6.922,P<0.05).Before treatment,there was no statistically significant difference in the absolute value of eosinophils between the two groups(t=0.023,P=0.818);after treatment,the study group was lower than the control group,and the difference was statistically significant(t=5.809,P<0.05).Conclusion During the treatment of patients with COPD with elevated eosinophils,the treatment of methylprednisolone combined with montelukast sodium tablets can not only improve the patient's lung function and inflammation,but also promote the improvement of the treatment effect.
