螺旋CT诊断急性阑尾炎合并周围脓肿的临床应用

Clinical Application of Spiral CT in Diagnosing Acute Appendicitis with Peripheral Abscess

目的分析螺旋CT诊断急性阑尾炎合并周围脓肿的临床应用价值。方法该研究方便选取2018年7月—2020年7月期间的70例急性阑尾炎患者进行了分析,患者经手术病理证实,对所有患者进行螺旋CT检查和B超检查,整理诊断结果,并与手术病理结果进行对比。结果CT诊断提示单纯性阑尾炎15例,化脓性阑尾炎30例,坏疽及穿孔性阑尾炎16例,阑尾周围脓肿7例,其他2例,分别占比21.43%、42.86%、22.86%、10.00%、2.86%,数据较之病理学检查结果均差异无统计学意义(χ^(2)=0.041、0.029、0.041、0.000、0.341,P>0.05);患者对CT检查的各项满意度评分分别是检查时间(30.22±2.31)分、等待时间(30.12±3.28)分、检查结果(30.23±2.43)分,较之超声检查结果差异有统计学意义(t=10.971、5.709、10.738,P<0.05)。结论螺旋CT与B超检查结果与病理诊断结果相比较,螺旋CT检出率及患者对检查过程的满意度较高,值得在临床中推广。

Objective To analyze the clinical application value of spiral CT in the diagnosis of acute appendicitis with peripheral abscess.Methods The study conveniently selected 70 patients with acute appendicitis from July 2018 to July 2020.The patients were confirmed by surgery and pathology.All patients were examined by spiral CT and B-ultrasound,and the diagnosis results were sorted and compared with the pathological results of surgery.Results CT diagnosis revealed 15 cases of simple appendicitis,30 cases of purulent appendicitis,16 cases of gangrene and perforating appendicitis,7 cases of appendix abscess,and the other 2 cases,accounting for 21.43%,42.86%,22.86%,10.00%,2.86%,there was no statistically significant difference between the data and the pathological examination results(χ^(2)=0.041,0.029,0.041,0.000,0.341,P>0.05);the patients’satisfaction scores for the CT examination were:examination time(30.22±2.31)points,waiting time(30.12±3.28)points,and inspection results(30.23±2.43)points.Compared with the ultrasound examination results,the difference was statistically significant(t=10.971,5.709,10.738,P<0.05).Conclusion The results of spiral CT and B-ultrasound are compared with the results of pathological diagnosis.The detection rate of spiral CT and the satisfaction of patients with the examination process are higher,which is worthy of clinical promotion.
