肿瘤标志物联合检测在胆管癌患者的诊断价值分析

Analysis of the Diagnostic Value of Combined Detection of Tumor Markers in Patients with Cholangiocarcinoma

目的分析肿瘤标志物联合检测在胆管癌患者的诊断价值。方法随机选择2019年7月—2020年7月该院收治胆道疾病患者50例,并按照病变性质分胆管癌组25例与胆管良性病变组25例,同时选取该院接受健康体检者30名为对照组,对3组的血清糖链抗原19-9(CA19-9)、糖链抗原242(CA242)、细胞角素片段抗原21-1(CYFRA21-1)以及血清细胞间黏附分子-1(sICAM-1)指标进行比较。结果胆管癌组肿瘤标志物阳性率最高,其中CA19-9阳性率为91.34%,sICAM-1与CYFRA21-1则在胆管癌组比较CA242和CA19-9阳性率低,而sICAM-1、CYFRA21-1、CA19-9指标均比胆道良性病变组与对照组高,差异有统计学意义(P<0.05);4种标志物浓度均超过正常值,其中胆管癌组的CA19-9、sICAM-1指标浓度最高,3组的血清标志物差异有统计学意义(F=256.150、100.677、51.741、101.667,P<0.05)。结论将CA19-9、CYFRA21-1、sICAM-1、CA242等肿瘤标志物联合检测应用于胆管癌诊断中,其诊断价值显著,可为临床早诊断和早治疗提供有效依据,临床上值得推广使用。

Objective To analyze the diagnostic value of combined detection of tumor markers in patients with cholangiocarcinoma.Methods From July 2019 to July 2020,50 patients with biliary tract disease were randomly selected to this hospital,and 25 patients in the cholangiocarcinoma group and 25 patients in the bile duct benign lesion group were divided according to the nature of the disease.At the same time,30 patients who received healthy physical examination in this hospital were selected as control group,for 3 groups of serum sugar chain antigen 19-9(CA19-9),sugar chain antigen 242(CA242),cytokeratin fragment antigen 21-1(CYFRA21-1)and serum cell adhesion numerator-1(sICAM-1)index was compared.Results The positive rate of tumor markers in the cholangiocarcinoma group was the highest,of which the positive rate of CA19-9 was 91.34%.The positive rates of sICAM-1 and CYFRA21-1 were lower in the cholangiocarcinoma group compared with CA242 and CA19-9,while the positive rates of sICAM-1 and CYFRA21-1.The indexes of CA19-9 were higher than those in the biliary tract benign lesions group and the control group,and the difference was statistically significant(P<0.05);the concentrations of 4 markers were all above the normal value,among which the index concentration of CA19-9 and sICAM-1 in the cholangiocarcinoma group was the highest,and the difference in serum markers of the three groups was statistically significant(F=256.150,100.677,51.741,101.667,P<0.05).Conclusion The combined detection of CA19-9,CYFRA21-1,sICAM-1,CA242 and other tumor markers in the diagnosis of cholangiocarcinoma has significant diagnostic value and can provide an effective basis for clinical early diagnosis and early treatment.It is clinically worthy of promotion and use.
