卵巢子宫内膜异位囊肿的CT诊断价值与影像学特点研究

Study on CT Diagnostic Value and Imaging Features of Ovarian Endometriotic Cyst

目的探讨卵巢子宫内膜异位囊肿(OE)采用CT诊断的价值及影像学特点。方法选择2018年2月—2020年2月该院接收的100例卵巢囊性病变患者为研究对象,均予CT、MR诊断。分析诊断结果、CT表现,比较两者诊断效用。结果①诊断结果:子宫内膜异位囊肿39例,成熟型畸胎瘤26例,浆液性囊腺瘤17例,单纯性囊肿10例,黏液性囊腺瘤8例;15例子宫内膜异位囊肿双侧发病,5例成熟性畸胎瘤,1例单纯性囊肿和1例浆液性囊腺瘤。子宫内膜异位囊肿双侧发病率15.00%高于单纯性囊肿1.00%、浆液性囊腺瘤1.00%、成熟性畸胎瘤5.00%,差异有统计学意义(χ^(2)=13.315、13.315、5.556,P<0.05);子宫内膜异位囊肿长径(4.69±1.51)cm低于黏液性囊腺瘤(12.29±3.61)cm、浆液性囊腺瘤(7.08±1.92)cm、成熟畸胎瘤(5.79±1.21)cm,差异有统计学意义(t=9.849、5.008、3.106,P<0.001);②诊断效用:CT诊断符合率90.00%与MR诊断符合率90.00%一致,差异无统计学意义(χ^(2)=0.000,P>0.05);③CT表现:39例子宫内膜异位囊肿中,20例“卫星囊”,10例囊壁增厚,4例囊内高密度影病灶,5例囊性包块,合并粘连7例。3例假阴性,其中囊壁增厚、粘连、囊性包块各1例。结论OE经CT鉴别诊断的价值与MR相近。

Objective To explore the diagnostic value and imaging features of ovarian endometriotic cyst(OE)by CT.Methods A total of 100 patients with ovarian cystic lesions received in the hospital from February 2018 to February 2020 were randomly selected as the research objects,and all were diagnosed by CT and MR.Analyzed the diagnosis results and CT findings,and compare the diagnostic utility of the two.Results 1.Diagnosis results:39 cases of endometriotic cysts,26 cases of mature teratomas,17 cases of serous cystadenoma,10 cases of simple cyst,8 cases of mucinous cystadenoma;15 cases of intrauterine cyst membranous cysts developed bilaterally,including 5 mature teratomas,1 simple cyst and 1 serous cystadenoma.The bilateral incidence of endometriotic cysts was 15.00%higher than simple cysts 1.00%,serous cystadenoma 1.00%,mature teratomas 5.00%,the difference was statistically significant(χ^(2)=13.315,13.315,5.556,P<0.05);the long diameter of endometriotic cyst(4.69±1.51)cm was lower than that of mucinous cystadenoma(12.29±3.61)cm,serous cystadenoma(7.08±1.92)cm,mature teratoma(5.79±1.21)cm,the difference was statistically significant(t=9.849,5.008,3.106,P<0.001);2.Diagnostic utility:CT diagnosis coincidence rate of 90.00%was consistent with MR diagnosis coincidence rate of 90.00%,the difference was not statistically significant(χ^(2)=0.000,P>0.05);3.CT findings:among 39 cases of endometriotic cysts,20 cases of"satellite capsule",10 cases of cyst wall thickening,4 cases of high-density lesions in the cyst,5 cases of cystic masses,7 cases of combined adhesions.3 cases were false negatives,including 1 case each with thickening of cyst wall,adhesion,and cystic mass.Conclusion The diagnostic value of OE by CT is similar to that of MR.
