摘要
目的探讨经腔内三维超声成像在子宫内膜息肉中的诊断价值。方法便利选取2017年4月—2019年6月收诊的70例疑似子宫内膜息肉患者,均行腔内三维超声。以病理组织学检查进行参照比较,观察疑似患者的确诊率,并分析其诊断满意度。结果经腔内三维超声检测,70例疑似子宫内膜息肉患者中诊断出67例子宫内膜息肉者,漏诊3例。而经病理组织学检查,3例漏诊者均为子宫内膜增生。70例疑似子宫内膜息肉患者的腔内三维超声确诊率95.71%,虽然略低于病理组织学的100.00%,差异无统计学意义(χ^(2)=1.363,P=0.243)。共存在37例单发病例,33例多发病例;经腔内三维超声检测,回声相对较强,能够对三角形边缘(基底层)进行清晰显示,其内膜线(腔内切面)较为整齐;若直径<15 mm,多会对成点状、条状的彩色血流信号进行探查,其基底相对更为狭窄。另外,腔内三维超声检查完成,患者的诊断满意率为97.14%。结论经腔内三维超声检查,能够对疑似子宫内膜息肉患者的子宫内膜腔形态进行全方位观察,其成像具有直观性与立体性,且其与病理组织学检查的确诊情况基本相似,在诊断效果、满意度方面均较佳。
Objective To explore the diagnostic value of transluminal three-dimensional ultrasound imaging in endometrial polyps.Methods Conveniently selected 70 patients with suspected endometrial polyps admitted from April 2017 to June 2019,all underwent intracavitary three-dimensional ultrasound.Use histopathological examination for reference and comparison,observe the diagnosis rate of suspected patients,and analyze their diagnosis satisfaction.Results Through intracavitary three-dimensional ultrasound detection,67 cases of endometrial polyps were diagnosed in 70 patients with suspected endometrial polyps,and 3 cases were missed.After histopathological examination,the 3 cases of missed diagnosis were all endometrial hyperplasia.In 70 patients with suspected endometrial polyps,the diagnosis rate of intracavitary three-dimensional ultrasound was 95.71%.Although it was slightly lower than 100.00%of histopathology,the difference between the two was not statistically significant(χ^(2)=1.363,P=0.243).There were 37 single-morbidity cases and 33 frequently-morbidity cases.The echo was relatively strong after intracavitary three-dimensional ultrasound detection,which can clearly display the triangle edge(basal layer),and its endometrial line(intracavitary cut surface)was relatively neat;if the diameter was less than 15 mm,and the color blood flow signals in dots and stripes were mostly detected,and the base was relatively narrow.In addition,the intracavitary three-dimensional ultrasound examination was completed,and the patient's diagnostic satisfaction rate was 97.14%.Conclusion Transluminal three-dimensional ultrasound can observe the morphology of the endometrial cavity in patients with suspected endometrial polyps.The imaging is intuitive and three-dimensional,and its diagnosis is basically similar to histopathological examination.It is better in terms of diagnostic effect and satisfaction.
